GENERAL FUNCTION:

Responsible for providing case management and supportive services to assist homeless households in coping with the challenges they face while transitioning from homelessness to permanent housing. The Rapid Re-Housing Case Manager Lead will also support the Case Manager with trainings, coaching, case consultations, and problem solving.

PRINCIPAL ACTIVITIES:

  • Assist the Program Manager with developing and providing initial and ongoing training for Case Manager.
  • Assist Rapid Re-housing team in case analysis to identify and implement appropriate interventions and referrals.
  • Main point of contact for all Foundational Community Supports (FCS) activity within Rapid Re-housing program, with collaboration with VOAWW Disability Services to ensure staff support and training, quality service delivery to clients, and efficient and accurate reporting.
  • Provide effective guidance and motivate team members as needed.
  • Act proactively to ensure smooth team operations and effective collaboration.
  • Create an inspiring team environment with an open communication culture.
  • Listen to team members’ feedback and resolve any issues or conflicts.
  • Recognize high performance and reward accomplishments.
  • Support Program Manager with developing and implementing systems and best practices to further support Case Manager. Model problem-solving for clients and team members.
  • Ensure that all individuals and households receiving Rapid Re-housing services meet program eligibility requirements.
  • Meet with individual/households to verify final eligibility and assist them in completing all necessary paperwork to facilitate entrance into the program.
  • Provide intensive housing search and placement assistance, which includes the services and activities needed to assist participants in locating and obtaining suitable permanent housing (assessment of needs and barriers to housing placement, negotiation with property owners/landlords to convince them to accept tenants they would normally screen out, ensure housing safety standards, and rent reasonableness).
  • Develop strong collaborative relationships with property owners/landlords to help facilitate the placement of clients into safe, decent, and affordable permanent housing.
  • Ensure clients understanding of rental agreements, housing policies and procedures.
  • Collaborate with other departments and partner agencies to maximize participant outcomes, program goals, and agency mission.
  • Determine and recommend the level and duration of financial assistance for eligible participants.
  • Prepare Requests for Payments for financial assistance and submit all required documentation to the Manager for approval.
  • Work closely with Manager regarding project spend downs (move in costs, rent assistance, etc.)
  • Maintain and manage all client documentation related to Rapid Re-housing.
  • Participate in all mandatory external and internal meetings and trainings.
  • Adherence with the Agency's code of ethics and regulations for confidentiality.
  • Develop proficiency in maintaining accurate client records in HMIS within the 5-day timeline as required by Snohomish County.

CASE MANAGEMENT ACTIVITIES:

  • Provide trauma informed, culturally appropriate, and client centered services.
  • Conduct weekly face-to-face or via phone case management meetings as needed.
  • Developing and monitoring an action plan with clients for locating housing.
  • Provide a minimum of one monthly home visit with participants.
  • Complete chronological progress notes within 24 hours of contact with client or collateral encounter.
  • Develop initial service plan specific and tailored to the client needs identified during initial intake assessment/screening process.
  • Revise or update services plans at least every three months or based upon the client level of need.
  • Completion of referral documentation and informed consent for supportive services.
  • Routinely assess strengths and barriers to housing stability (including health, substance use and mental health issues), and provide appropriate referrals to supportive services in a timely manner. Support with developing income increase strategies, money management, employability, and wage progression.
  • Timely completion of Fenn-Jorstad Self-Sufficiency Matrix to assess participants’ sufficiency level across multiple life domains.
  • Complete assessments for housing retention and long-term stability during this program participation, and after graduation.

QUALIFICATIONS:

  • BA in Human Services or a related field, plus a minimum of 3 years of experience working directly with homeless populations.
  • Experience working with clients that have mental illness and substance abuse challenges.
  • Experience working with families and individuals in crisis.
  • Decision-making skills.
  • Leadership skills.
  • Ability to provide culturally appropriate services to people from diverse backgrounds.
  • Effective organizational skills, and attention to detail and accuracy
  • Highly developed interpersonal skills with the ability to work cooperatively with staff, landlords and participants
  • Ability to prioritize and work independently
  • High proficiency in MS Office programs, especially Excel
  • Ability to work in the office, or remotely as needed
