Summary:
The Annual Wellness Visit (AWV) Navigator assists by working directly with patients to offer timely coordination of care and support; education regarding AWVs; appointment options; direct patient care and care plans. In addition, the Navigator communicates with physicians, support services, and family members to ensure patients receive value-based care as needed.

The Navigator will provide AWV appointments to patients in need of their annual visits.

Responsibilities:
Serves as the point of contact for patients and families to assist with understanding, scheduling and providing direct patient care as it relates to Annual Wellness Visits.

Interacts with office personnel to assure proper scheduling of these visits.

Communicates with PCP regarding any patient care issues resulting from AWV.

Maintains proper documentation to ensure compliance with clinical assessment and needs.

Follows up as necessary

Provides direct patient care as needed to patients of the practice.

Coordinates the clinical and administrative activities related to the office to ensure the delivery of quality, cost-effective patient care.

Examines, evaluates, diagnoses and treats patients to provide expeditious, high quality, customer focused care.

Interprets and integrates data to determine the appropriateness of the therapeutic treatment plan.

Consults with supervising physicians in accordance with the practice's policies and medical standards to assure standards of medical care and patient satisfaction.

Performs total quality improvement processes and initiatives to achieve the Practice's mission.

Responsible for following departmental policies and procedures to assure service with proper risk management.

Fulfills all compliance responsibilities related to the position.

Performs other duties as assigned.
