THL Care Coordinator Case Manager Job at Ridgeview Behavioral Health Services
RIDGEVIEW Behavioral Health Services, a not-for-profit community mental health center in Harriman, TN, has an immediate opening for a Full Time Bachelor's Level Case Manager/ Care Coordinator to provide social services in the Morgan and Roane County communities.
The Care Coordinator position is part of the THL (TN Health Links) program that works to support clients in reaching mental health and medical health goals. Successful candidates will have a passion for serving clients and supporting Ridgeview's mission of Hope, Healing, and Recovery through the provision of social services.

REQUIREMENTS
Bachelor's degree in social services (ex social work, psychology, sociology, counseling, vocational rehabilitation) or Bachelor's degree transcript that shows 15+ credit hours of related courses. Good organization and time management skills; documented safe driving record; the ability to work independently while interacting within a multidisciplinary team environment. Prefer experience working with individuals with severe and persistent mental illness. Must maintain a valid TN driver's license and a safe driving record.
JOB DUTIES
Care coordination and case management refers to a process for directing and coordinating services to consumers of mental health services. The THL Care Coordinator or Case Manager works directly with adult or youth clients, their families, and community resources for mental health both in and out of Ridgeview to secure necessary services to meet client needs. THL Care Coordinator clients include both adults and children.
