Busy medical practice in TUSCALOOSA is seeking a LICENSED SOCIAL WORKER. We pride ourselves on excellent patient care and service. We are seeking the right candidate who has a passion for the patients we serve in our community.

Education, Training, Experience, and other Requirements:

• Bachelor’s or Master’s degree in Social Work or behavioral health studies.
• MSW candidates are preferred to be LICSW eligible and obtain licensure within 6 month of hire date.

• 2 years of experience in the field or in related work experience preferred.
• Ability to make professional evaluations, decisions, and recommendations for treatment planning and implementation.
• Must have the ability to work independently and in a team setting.
• Excellent verbal and written communication skills.

Summary:

• Provides assessments, develops care plans, help clients identify goals, develop therapeutic process, and have a holistic approach to mental health care.
• Complete and maintain documentation in a timely and accurate manner and according to Federal, State, and agency guidelines.
• Provides follow-up via on-going assessment, evaluations progress toward defined goals, amends care plan as needed and address issues of patient non-compliance.
• Provides patient referral to for financial assistance, medical specialties, shelters, vocational services, legal services, and substance abuse treatment providers, Social Security Administration, Food Stamp Office, Medicaid, and other available resources.
• Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.
• Complete and maintain documentation records of all client assistance and follow-up.
• Utilizes the electronic medical record system (EMR) in documenting visits.
