We are working with a reputable Home Healthcare Agency in Broward, seeking a Field RN / Case Manager. The ideal candidate will report to the Director of Nursing and will be responsible for proactively applying professional knowledge and skills to meet the needs of the patients in the field and facilitate the achievement of individualized, defined and measurable outcomes.

MAIN RESPONSIBILITIES:

  • Provide skilled nursing interventions in the treatment of the patient illness, rehabilitative needs and preventative care in their homes or wherever they reside
  • Provide services in accordance with accepted standards of clinical practice, to facilitate problem solution, and thereby, the achievement of individualized patient goals and outcomes.
  • Accurately and thoroughly documents patient’s care observations, interpenetrations and evaluation on a day services are rendered.
  • Assures that verbal orders obtained from the physician are accurately transcribed for implementations.
  • Documents supervisory visits for inclusion in the patient’s records on the prescribed form in a timely manner.
  • Monitors the appropriate completion of documentation by LPN’s / home health aides / nursing assistant as part of the supervisory /leadership responsibility.
  • Contact the office / DON immediately with any itinerary changes.
  • Can be contacted at all times by beeper / cellular while in the field.
  • Participates in the Quality Assessment and Improvement Activities as assigned. Communicates information that enables collection and root analysis of data to identify opportunities for improvement.
  • Attends, participates in and/or conducts internal staff development programs, obtains continuing education as required by Agency policy and regulations.
  • Participates, as assigned, in teaching and training of other healthcare staff members including, but not limited to: a. Orientation of staff members to an individualized plan of care, inclusive of expected goals and outcomes. b. Providing leadership and direction, especially as related to paraprofessional personnel, in the ongoing implementation of the plan of care.
  • Timely assessment and provision of policy or procedure specific training whenever a needed is defined.

REQUIREMENTS:

  • Must be a graduate from an accredited school of nursing.
  • Valid RN license in state of Florida.
  • Experience as an RN which complies with ONE of the following:
  • **One (1) year acute care experience within the last two (2) years;
  • **One (1) year home healthcare experience within the last two (2) years;
  • **One (1) year community health or public health experience within the last two (2) years;
  • **One (1) year outpatient facility experience within the last two years;
  • Credentials in specialty area, i.e., phlebotomy, IV, ET.
  • Current CPR card.
  • Current health certificate/physical examination and TB testing.
  • Valid state driver’s license and reliable vehicle with auto insurance
  • Full command of verbal and written communication skills effectively in English.
  • Demonstrated leadership capabilities.
  • Additional languages preferred.
  • Must be okay working 2 Saturday's per month.

Pay: $70,297.51 - $73,337.43 per year
