RN Case Manager Job at Bristol Hospice L.L.C
Bristol Hospice is growing yet again, and looking to add an additional RN Case Manager to our Honolulu team!
In this role the RN will independently schedule patient visits, create care plans, and utilize their interdisciplinary team to create the best care they can for their patients. This is a bedside nursing and bedside charting position with one on one patient care.
The Registered Nurse Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
SCHEDULE:
- Full Time, Monday through Friday
JOB REQUIREMENTS:
- Must be a licensed registered nurse (RN) with current state licensure in good standing
- Minimum one (1) year of RN experience.
- Must be a licensed driver with current auto insurance and automobile in good working order.
