About this Position

We Offer a full range of residential and clinical services that provide support, rehabilitative care, and skills training to foster independence for individuals affected by mental illness. This position is responsible for providing supportive counseling, crisis intervention, restorative services and providing training in independent living skills. This position is scheduled for Mondays- Fridays from 3pm-11pm coverage.

ROLE AND RESPONSIBILITIES:

  • Residential Counselor will meet with assigned residents at least once a week.
  • Participates with the residents in the development, evaluation and modification of individual service plans in achieving their goals.
  • Responsible for completing all required documentation including progress notes in a timely manner.
  • Assist residents in maintaining financial entitlements.
  • Trains and assists residents in the household with activities of daily living skills, including meal planning and personal care activities.
  • Provides crisis management and case management service.
  • Provides transportation as needed, for food purchases or shopping, medical appointment etc.
  • Conducts weekly house inspections within the group home to determine what repairs are needed to ensure a clean and safe home environment. Reports problems to appropriate personnel.
  • Monitor medication as required.
  • Notifies the appropriate supervisory personnel of any medical, psychiatric, or programmatic emergency.
  • Screens calls from apartment residents, assesses situation and, if necessary, contacts the on- call person for further direction.

Qualification/Education Requirements:

  • Minimum High School Diploma with 5 years' experience required.
  • Bachelor’s degree in human service field preferred. However qualified experience may be substituted for Associates Degree or Bachelor's Degree.
  • Demonstrates organizational and communication skills which would enable effective interaction with agency staff, consumers/clients/families and the community.
  • Critical thinking and sound judgment skills are necessary.
  • Close in-person interaction with clients, staff and/or outside representatives.
