The Job

Provides leadership to expand SCAN offerings and capabilities for high quality population health programs. Innovates and implements new clinical programs for members with emerging or complex health issues, behavioral and mental health care needs, and end of life care. Shapes sustainable programs to improve member experience and outcomes.


You Will

Works with the SCAN Value Based Care (VBC) team and the extended Chief Medical Officer (CMO) leadership team to identify, develop, design, and implement new clinical programs to expand SCAN’s care model and to enable improved outcomes for its membership.

Works with senior management in VBC to develop strategies that enhance SCAN’s care model to deliver on the Triple Aim, focus on emerging risk and high-risk populations, and ensure healthcare equity within program design and interventions.

Identifies areas of improvement within SCAN’s care model to address unmet clinical, behavioral, social, and cognitive needs. Builds and sources specific programs by identifying populations to target, goals for improved care in populations, services that drive outcomes, and evaluation metrics that will help assess the programs. Implements new programs with a focus on business case development, outcomes measurement, and operational design.

Facilitates annual evaluations of SCAN programs serving emerging risk and high-risk members to update the portfolio of services offered.

Promotes quality with strategies and programs to support cost effective care and recommends appropriate 5-STAR measures that address clinical metrics and experience.

Interfaces with SCAN Complex Care Management and Utilization Management leadership to ensure scaling of programs and to support growth and expansion.

Achieves financial objectives by supporting the preparation of an annual budget, approving appropriate expenditures, analyzing variances, and initiating corrective action.

Ensures that SCAN systems support program delivery and that new performance measures are developed and tracked to evaluate program effectiveness. Supports the development and preparation of management reports that assess the effectiveness or programs and services.

Actively support the achievement of SCAN’s Vision and Goals.

Your Qualifications

  • Bachelor's Degree in Health Care field, Master's degree preferred
  • 5+ years management experience in a Managed Health Care or healthcare startup setting, Medicare/Medicaid
  • Strong healthcare program development, quality and data analysis skills including understanding of basic research methods, and ability to make decisions accordingly
  • Strong interpersonal skills, including excellent written and verbal communication skills.
  • Strong organizational skills; Ability to multitask.
  • Ability to appropriately maintain confidentiality.
  • Excellent presentation skills.
