Job Summary

This position is responsible for managing Coordinated Care Organization (CCO) initiatives and programs aimed at behavioral health within the regional health care delivery system, as well as partnering with Health Share partners to participate in a wide range of projects integrating behavioral health services into medical and other settings. Areas of focus include collaborations involving our regional county partners and their behavioral health and crisis safety net programs and how behavioral health benefits are integrated across the spectrum of Health Share’s health plan partners. This position provides support to CCO efforts to meet behavioral health metrics created by the Oregon Health Authority, as well as CCO contract deliverables and reporting required by the OHA. This position also establishes strong relationships with internal and external partners, facilitates cross-organizational workgroups to improve access to care, and supports broader medical and health plan initiatives within the Integration department at Health Share. Additionally, this position assists with complex member case resolution, as well as liaison to a broad range of care coordination and health system navigation teams.


Essential Responsibilities

Strategy Development and Collaboration

  • Support, contribute to, and author a range of required reporting related to Behavioral Health as part of our Oregon Health Authority CCO contract.
  • Develop relationships and monitoring strategies for Health Share funded behavioral health programs, such as county-based crisis teams, peer services, and complex care management teams.
  • Assist with monitoring access to key behavioral health programs and liaison to CareOregon behavioral health staff.
  • Work with CareOregon’s behavioral health team and our medical health plans to support CareOregon programs that serve the general CCO membership.
  • Provide strategic support for Behavioral Health programs and services to improve integrated care and outcomes for socially complex issues.
  • Support transformation within the CCO and our health plan partners as new Medicaid waiver initiatives are implemented.
  • Support programs that intersect behavioral health services with housing, health related services, and other social determinates.
  • Support Health Share’s CCO Incentive Metric work, working with health plans, analytic teams, and community organizations to achieve CCO goals.
  • Work with analytics staff to develop and refine behavioral health dashboards and associated reporting.
  • Facilitate and participate on workgroups comprised of health plans, providers, counties, and other community partners to move a wide range of behavioral health and related projects forward.
  • Support existing behavioral health venues, such as the Health Share Behavioral Health Advisory Council, Joint Behavioral Health Workgroup.
  • Consult with care coordination teams regarding complex behavioral health cases.
  • Liaison with Oregon Health Authority staff on escalated behavioral health cases.
  • Assist in representing Health Share to the Oregon Health Authority regarding behavioral health rules and processes that impact our CCO, health plans, and county partners.

Partnerships, Networking, and Communications

  • Establish and maintain professional collaborative relationships with health plans, behavioral health providers, the OHA, county behavioral health teams, community service agencies, etc.
  • Work with diverse groups of stakeholders to develop a community wide approach to improving behavioral health access and integration.
  • Facilitate strategic communication within Health Share partner organizations and state and local systems to explore cross-system partnerships in support of critical populations.
  • Support strategic partnerships within the CareOregon Behavioral Health benefit to move work forward that improves access, quality, and coordination of care.
  • Participate in the development and implementation of dashboards to monitor disparities and improvements at the population level.

Project Management and Coordination

  • Ensure alignment of project outcomes with the CCO’s commitment to racial equity.
  • Work with highly matrixed teams assigned to collaborate on the planning and implementation of projects impacting multiple stakeholders.
  • Collaborate with partners to define project goals, objectives, milestones, and budgets to support identified initiatives; monitor progress to ensure initiatives achieve intended results.
  • Develop contracts including statements of work, budgets, and reporting requirements and monitor progress towards goals.
  • Coordinate efforts with related initiatives, using a system integration approach.
  • Manage and support strategic projects and initiatives based on emerging priorities.
  • Develop and sustain a project plan with clear milestones, timelines, and risks; track progress to goals and communicate status updates to stakeholders.
  • Analyze, document, and communicate project outcomes and lessons learned.

Knowledge, Skills and Abilities Required

Knowledge:

  • Knowledge in the areas of Medicaid behavioral health providers, county and community mental health programs, health plan processes and regulation, Trauma Informed Care, and care coordination processes
  • Knowledge related to CCOs and the role they play in Oregon’s Medicaid environment

Education and/or Experience

Required:

  • Minimum 5 years’ experience managing projects, programs, or initiatives in a health care or public health setting
  • Experience building relationships with and defining solutions with external partners and stakeholders

Preferred:

  • Experience working with state/local Medicaid systems
