Position: Population Health Analyst - PRIME

Compensation

The estimated pay for this position is $29.3252 to $36.0663. The rates shown include a 6% premium pay (base= $-$ plus 6%). This reflects only a portion of the total compensation package for this position. Additional compensation may be available for this role through differentials, incentives, and bonuses. In addition, this position may be eligible for participation and company contributions to the Kern County Employees’ Retirement Plan.

Definition:

Under the direction of the Director of Population Health, the Population Health Analyst performs a wide variety of analytical and technical support to leaders and staff of population health programs. These programs include the usage of the healthcare system, behavior health and social needs. In addition, assists in all functions associated with the Authority’s healthcare transformation and integration-based initiatives.

Distinguishing Characteristics:

This position is distinguished by its focus on utilizing data analytics to improve population health and support quality improvement initiatives. This Analyst will develop, deploy, validate, track and maintain analytic models.

Essential Functions:

  • Assist in the development of protocols and methodologies that support the use of informatics practices that integrate clinical health, environmental health and population health data. This includes identifying internal and external data sources, procuring and manipulating data and documenting the process.
  • Responsible to analyze and aggregate data across multiple technology resources to bring together in an actionable format aimed at achieving and maintaining compliance with Federal and State quality improvement incentive programs such as Medi-Cal waivers, Meaningful Use and MIPS.
  • Responsible to work with cross functional teams to develop patient centered data models.
  • Enhance data collection procedures to capture critical information for building analytic systems.
  • Responsible to prepare comprehensive, clear and concise statistical and administrative reports.
  • Assists in the development of organization-wide data stewardship practices, promoting accuracy, engagement, and standardization of reporting methodologies.
  • Understands the core components of team-based care and other patient-focused care methodologies (e.g. Patient-Centered Medical Home), inclusive of mental/behavioral health integration and complex care management.
  • Understands quality and performance improvement tools and methodologies (e.g. Lean Six Sigma), and utilizes the same towards improving both patient care and operational processes.
  • Develops and maintain relationships with key stakeholders to facilitate effective project design and implementation.
  • Promotes the mission, vision, and values of the Kern County Hospital Authority.

Employment Standards:

  • Completion of a Bachelor’s Degree from an accredited college or university in Healthcare Administration, Public Health, Statistics or related field is preferred. Prior experience with healthcare data is desired.

Knowledge, Skills and Abilities:

  • Sound quantitative analytical and problem-solving skills.
  • Demonstrated ability to use computers and computer applications, including word processing, spreadsheets, and database applications.
  • Experience working with complex data sets and the ability to present data in a way which facilitates action.
  • Demonstrated ability to communicate effectively, both verbally and in writing, with results that are professional in both content and delivery.
  • Demonstrated ability to work in a team-based environment and establish and maintain cooperative work relationships.
