The Performance Improvement Specialist – PACE is responsible for creating, directing, coordinating and evaluating a Performance Improvement plan that meets, at a minimum, the requirements under Title 42 CFR § 460, Subpart H – Quality Assessment and Performance Improvement (QAPI). Quality monitoring and performance improvements must be established for the multiple regulated and un-regulated medical, institutional and community based settings necessary to providing quality, person-centered and cost-effective care consistent with the National Program of All-inclusive Care for the Elderly (PACE) model. Care settings include the adult day care center, Keyser Clinic, contracted assisted living facilities, skilled and unskilled in-home care, contracted acute facilities, skilled and long-term nursing facilities, and transportation services.

This position is responsible for establishing and monitoring quality and performance metrics that support the clinical and insurance functions of a PACE program as they affect participant safety and satisfaction. Responsible for ensuring compliance with federal and state regulatory quality reporting.

Reports to the Director, Hopkins ElderPlus and takes direction from the Medical Director in the development of quality goals. Member of the Hopkins ElderPlus Management Team.

    • Assures that the quality monitoring and improvement activities are compliant with PACE regulations, CMS audit guidelines and Johns Hopkins Bayview guidelines.
    • Responsible for the development of the Performance Improvement (PI) plan for submission to the JHBMC, JHHC and JHHSC Boards of Trustees, and submitted to CMS via the CMS Health Plan Monitoring System (HPMS).
    • Prepares and presents monitoring reports to the Hopkins ElderPlus Quality Committee (JPC) to support analysis of progress towards goals of the PI plan.
    • Establishes, coordinates and provides direction to interdisciplinary project improvement teams and rapid cycle improvement workgroups using Lean management and similar techniques.
    • Reports required CMS quality measures through HPMS. Completes root cause analysis on all incidents for which CMS requires a root cause analysis.
    • Conducts and/or coordinates regular chart audits as required to validate monitoring reports and meet CMS audit targets.
    • Manages root cause and rapid cycle improvement projects as needed to maintain compliance with quality standards.

Hours:

  • Full time, 40hrs per week
  • Mon. – Fri., 8:30a – 5:30p

Work Experience/Requirements:

    • Work requires demonstrated ability in successfully developing and leading interdisciplinary clinical and/or performance improvement teams.
    • Work requires the level of knowledge of health care delivery systems typically acquired through completion of a Master’s Degree in Public Health Administration, or closely related field such as Health Policy, Business Administration, Health Administration, or 3-5 years previous experience in leading quality assurance and performance improvement in clinical and/or managed care settings.
    • Master’s degree or Masters Candidate required in the fields Public Health, Health Policy, Business Administration, Health Administration, or a field related to quality engineering or business process improvement.
    • Annual Certification in CPR is required.
    • Work requires the analytical skills to resolve problems that require the use of basic scientific, mathematical or technical principles. Must have documented experience using Excel to design reports and use of analytical tools to collate and analyze data.
    • Work requires the ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.
