Midland Care's PACE Center in Topeka is hiring for an RN Case Manager!
Midland Care is looking for an experienced RN to join our PACE team as an RN Case Manager. Midland Care RN Case Managers participate as a member of the interdisciplinary team to assess, plan, implement and evaluate care provided in the patient/participants place of residence.
Employment Status: Full-Time | Monday-Friday | 8am-5pm
$3,000 Sign on Bonus!
Pay Rate starting at $66,000 (based on experience in role)

Qualifications
Required: Current unrestricted State of Kansas License to practice as a Registered Nurse. Availability of reliable transportation is required. Knowledge of professional nursing theory, standards of nursing practice, scope of practice and relevant statutes and regulations. Ability to prioritize work, meet deadlines, handle multiple projects, pay strong attention to detail and accuracy and work under pressure in a cooperative work environment. Must be self-directed, motivated and flexible. Must possess a valid Kansas Driver’s License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization’s insurability standard as defined by Midland policy. Availability of reliable transportation is required for this position.
Preferred: Home Health or Hospice experience preferred. One year professional medical-surgical experience preferred.
