LPN Intake Coordinator- Bloomington, IL Job at CRS & Home Health Advantage
Home Health Advantage is recruiting an LPN for Intake Coordination role. You will be expected to manage the referral process: Verify home health orders, primary care physician, identify face to face and progress notes, review all referral documents, create a care summary for each new patient, verify staffing and provide an intake summary at huddles twice per day. This is an office based position and you will be collaborating with office and marketing team members in person and via phone. Position can be based in Orland Park, IL or Bloomington, IL .
Must be LPN licensed in state of IL. Must be able to summarize clinical scenario and write clinical summaries. Must be able to expedite the referral process efficiently and effectively.
