The Support Services Manager is responsible for providing the support, training, information and resources in a variety of support models we provide to a diverse population of adults with developmental disabilities and ASD living in different in various independent living settings and the staff that support them. Shared Living providers who make the provision of quality services possible. This position also includes the responsibilities of providing direct support and assisting with the hiring and ongoing supervision of other direct support staff/ consultants for specific contracted support services for the department. The Manager’s responsibilities consist primarily of advocating for the individuals SL providers and additional support staff and consultants, since without the existence of all team members, there would be no service support. The Manager is directly accountable to the Director of Individualized Support Options.

ESSENTIAL FUNCTIONS:

  • Obtain social history and conduct psychosocial assessments and evaluations of all individuals referred for placement.
  • Selecting, training, evaluating and supervising providers, support staff and consultants as needed but at least one time per month.
  • Participating in assessments, development, implementation and ongoing review of participant’s Individual Support Plan (ISP).
  • Conducting on site visits with participant and provider at qualified setting bimonthly/monthly to ensure quality of care and ongoing appropriateness of placement.
  • Completing care manager progress notes on each visit and documentation of any significant change.
  • Assisting with obtaining information and accessing other services in order to enhance supports.
  • Provide direct support to individuals assigned to specific support service models as required.
  • Conduct annual recertification assessments for each provider home.
  • Conduct annual provider/ staff performance evaluations.
  • Coordinate provider/ staff support groups, in-service trainings, and special events with providers/individuals.
  • Develop goals and objectives with the provider, and, when appropriate, the individual. Complete semi-annual goals and objectives; participate in annual reviews and other team meetings.
  • Coordinate and/or make referrals for ancillary services for individuals, in cooperation and conjunction with other professionals involved in the case.
  • Recruit new providers, and conduct home studies, check references, and orient new providers.
  • Coordinate and participate in pre-placement visits, transition visits and family visits as needed.
  • Complete monthly support services including monitoring service hours and billing.
  • Coordinate other support services which are needed: relief, skills training, and other supports.
  • Work with the Director of Individualized Support Options on any special projects which may arise and are deemed appropriate.
  • Attend orientation, in-service trainings and regularly scheduled supervision meetings.
  • Make suggestions and recommendations as to the hiring, firing, advancement, promotion or any other change of status of other employees.
  • Adhere to the implementation of all additional oversight of SL placements required as specified for homes identified as placements in accordance with program regulations.
  • Be available to respond to all emergencies and crises as needed.

QUALIFICATIONS:

A BA/BS in Human Services or related field and 5 years of experience working with individuals who have developmental disabilities and high Functioning ASD. A valid driver’s license and car are required. This position may involve physically assisting individuals. Good written and oral skills are required to effectively communicate.

Job Type: Full-time

Pay: $22.00 - $26.00 per hour
