ABOUT US: Rogers' Consulting and Management is an Independent Living Service agency committed to improving the quality of life for adults with developmental disabilities (Consumer). Through the Individual Personal Plan process, these individuals have chosen to live on their own or with family with the goal of moving out on their own.

MISSION: Rogers' Consulting and Management (RCM) mission is to improve the quality of life for seniors, adults, and families impacted by disabilities and special needs.

We accomplish this mission through advocacy, education, community partnerships, and an array of innovative, customized services, supports, and training so that individuals can reach their full potential throughout their lives.

RCM's Independent Living Services program for seniors and adults with disabilities respects each person's right of self-determination, choice, and control. We work to support the choices of men and women to pursue activities and work that they find meaningful, enriching, and interesting to reach their maximum potential for independence.

SCOPE: Provides direct services to consumers; Coordinates, completes, and assessments and progress reports; Implements individual service plans. Serves as an advocate and liaison for and on behalf of the consumer. Formulates and updates reports on educational and service delivery systems

SUPERVISION: Receives general supervision from the Program Services Supervisor

EXAMPLES OF (These are examples of the types of duties that may be performed) Additional DUTIES: duties may be added)

  • Follows the Program Plan of individual, including working towards individual goals
  • Has the ability to perform duties depending on the level of care of the Individual
  • Fills out all required documentation at the end of each shift
  • Maintains current progress notes and date frequency count on each client assigned to them
  • Attends staff meetings and in-service trainings
  • Communicates with other staff and program managers about Individual progress
  • Establishes and maintains a constructive and appropriate personal relationship with consumers
  • Participates in client's assessment, planning, and evaluation process
  • Participates in the development of the individual services plan
  • Provides reports on client progress as outlined in the Individual Program
  • Minimizes risks of endangerment to the health, safety, and well-being of consumers
  • Reports special incidents according to applicable laws and regulations

Assists with training/educating/empowering individuals with their daily living activities depending on the Individual's needs. These may include:

  • Meal Planning, Meal Preparation, and Cooking
  • Sorting, Rearranging, and Cleaning
  • Money Management and Budgeting
  • Using/Understanding Community Resources, i.e., SSI, IHSS, etc.
  • Exploring affordable and low-income housing
  • School assignments and educational programs
  • Scheduling and Attending medical, mental health, and dental appointments
  • Assist with medication dispersal if applicable for Individual
  • Participating in community events and activities
  • Providing support with hobbies, studies, and social activities
  • Personal Hygiene and Personal Development, if applicable for the Individual
  • Supporting the Individual with gaining/increasing work readiness, job coaching, and securing employment

SKILLS:

  • Maintains ability to exercise mature judgment and relate to clients in a warm, caring, and respectful manner
  • Has intermediate computer skills
  • Has strong problem-solving skills
  • Has strong critical thinking skills
  • Has solid written and verbal communication
  • Excellent emotional intelligence and ability to establish rapport with others

KNOWLEDGE:

  • Knowledge of Developmental Disabilities; Regional Center (or related field) practices
  • Knowledge of resources in the Antelope Valley, Santa Clarita, San Fernando

REQUIREMENTS:

  • Own a Smartphone
  • Own a working/functional computer with a camera
  • Speaks, reads, and writes well in English
  • Has a FLEXIBLE Schedule
  • Has a minimum of 2 years of verifiable experience providing direct care services to individuals with developmental disabilities
  • Has an Associate Degree, Preferable a Bachelor Degree
  • Has a valid First Aid/CPR Certification
  • Be able to pass an employment screening process
  • Has a valid Driver's License
  • Has reliable transportation and state-minimum auto insurance
  • Be able to pass TB test and Health Screening
