About This Nursing Position

HopeHealth is seeking an experienced Registered Nurse for its affiliate HopeHealth Hospice & Palliative Care. This position will report to the Providence office, and will provide services in northern Rhode Island Hospitals, particularly The Miriam Hospital.

This is a 40-hour/week position with 9:30am-6pm hours. There is a rotating weekend commitment every 3rd to 4th weekend. This position is eligible for full benefits.

The GIP Admissions Nurse (RN) provides skilled assessment of patients for eligibility for the hospice benefit in collaboration with the hospice medical director. The Admission RN ensures compliance with all Medicare/Medicare and State for hospice admission and initiates patient care as appropriate.

The Admissions RN serves as the first face-to-face contact for patients, families and caregivers. The Admissions RN can describe the services and philosophy to potential patients/families.

This position is eligible for a bonus of up to $2,500.


What will you do?

  • Represents HopeHealth at assigned acute care setting providing education on the hospice benefit and acts as a liaison with the facility staff.
  • Provide initial assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living to determine eligibility for the hospice benefit. Presents hospice in a manner to eligible patients that highlights the benefits of hospice in caring for individuals with a terminal diagnosis.
  • Create the individualized plan of care including documenting the Local Coverage Determination (LCD).Performs initial assessment of patient and family to determine the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living. Apply knowledge of LCD training to communicate assessment to the hospice Medical Director.
  • Uses interdisciplinary approach in developing plan of care, in accordance with the organization's policies. Initiates referrals to other disciplines and refers to community resources only when necessary to meet an immediate need.
  • Participates in weekly IDG meetings to review patients on GIP level of care.
  • Documents in EMR and meets department expectations for completing in a timely manner in order to provide optimal response to all patients.
  • For patients admitted to the GIP level of care, completes daily visits to assess patients and determined continued eligibility for GIP in collaboration with the Hospice Medical Director.
  • Coordinates care with facility staff providing regular updates, communication MD recommendations for care and informs of current hospice plan of care.
  • Ensures that patient information regarding payer source is current and correct. Works with the organization to obtain necessary insurance information and coverage.
  • Provides other duties as assigned, including but not limited to, filed evaluations/admissions, triage visits and GIP coverage at adjacent facilities.
  • Participates in weekend, evening shift and holiday coverage for HopeHealth


What are the qualifications for this job?

  • RN required BSN preferred
  • 1-3 years of related experience
  • Strong nursing assessment, clinical skills and communication skills
