We are a Denver based home health agency looking for a compassionate and dependable SLP to join our team. We are looking for individuals that understand the value of providing skilled care in the place people call home. We currently have needs for for our North (Adams/Broomfield/Boulder counties).

Although a multi-location company, we pride ourselves on maintaining a local, family-owned feel while providing exceptional care to those we have the honor of working with. Although this position is Monday through Friday from 8:30 - 5pm, our SLP role is autonomous with built in flexibility based on your patients’ needs. Required License and Qualifications:

Speech and Language Pathologist with current CO license

Maintains current CPR Certification that meets American heart Association BLS Guidelines

Licensed driver with automobile that is insured in accordance with state requirements

Able to lift, position, and/or transfer patients

Essential Job Functions / Responsibilities

Provide in-home and facility visits to assess patient eligibility and needs to initiate appropriate interventions

Develops care plan and teaches patient and caregivers care techniques as appropriate

Educate and prepare patients, families, and caregivers to managing their acute and chronic care needs to include: communication deficits, disease process management and education, cognitive deficits, swallowing difficulties.

Provide mental and emotional support to patients and families as appropriate

Collaborate with other members of inter-disciplinary team (Physical Therapist, Occupational Therapist, Speech Therapist, Social Worker, and CNA) providing patient care.

Collaborate and communicate with referral partners as appropriate

Complete accurate and relevant clinical notes in EMR

Job Type: Per diem

Pay: $35,000.00 - $72,000.00 per year

Pay: $40.00 - $70.00 per hour
