Linil Visiting Nurses, Inc. is looking for a Registered Nurse Case Manager who will be responsible for planning, coordinating, providing and documenting care for an assigned caseload of patients from admission to discharge. As a Registered Nurse Case Manager, you will develop and implement a home health plan that meets each patient’s specific needs.

Orange County, Seminole County (Apopka, Altamonte Springs, Orlando, Winter Park, Winter Garden, Winter Springs, Casselberry, Oviedo)

Lake County (Clermont, Tavares, Mt. Dora, Eustis, Leesburg)

Professional Requirements

1. Successful graduation from an accredited School of Nursing with an ASN or BSN degree.
2. Must be licensed-in the State of Florida as a registered nurse.
3. Experience as an RN commensurate with at least 2 years of: acute care experience; OR home health experience; OR skilled nursing facility/outpatient facility
4. Valid driver's license and auto insurance in the State of Florida.

Primary Duties and Responsibilities:
1. Performs a comprehensive assessment of patients and assesses their condition every visit.
2. Provides nursing care in accordance with the highest nursing standards and patient safety standards.
3. Performs an OASIS/comprehensive assessment of newly admitted patients and develops an individualized Plan of Care based on the admission assessment.
4. Implements the Plan of Care and re-evaluates it as necessary, including the need to re-certify patient care or discharge the patient when all goals are met.
5. Follows and upholds patients' rights, including treating all patients with dignity, respect and compassion.
6. Demonstrates knowledge of disease management, medication management. nursing procedures and risk/error reduction strategies, as well as the ability to apply such knowledge in the delivery of patient care.
7. Collaborates with the patient's physician as needed, and obtaining verbal orders to update the patient's care whenever necessary.
8. Collaborates with the entire care team as needed participating in case conferences when asked, attending staff meetings, participating in QAPI activities, and updating the clinical manager as necessary.
9. Completes educational in-services as required.

10. Represents the organization in a professional manner.

Job Types: PRN, Per diem, Part-time

Pay: $32.00 - $70.00 per hour
