HOURS: 8-5 M-F (flexibility a must)
CLASSIFICATION: Full-time/Hourly/Non-Exempt
PRIMARY LOCATION: 233 West Ferry (must be able to travel to multiple locations as needed)

JOB SUMMARY: *The HealthySteps Specialist is an early child development expert who will join the pediatric primary care team at Jericho Road Community Health Center to provide interventions, referrals, and follow-up for families with patients ages 0-3. The HealthySteps Specialist builds strong relationships with families and providers to increase the efficiency of the medical system and support team-based comprehensive care.

Essential Job Duties*

*
PREVENTATIVE CARE*

  • Build rapport and effectively engage with diverse clients about medical care demonstrating cultural competence in all interactions and opportunities
  • Perform and coordinate standardized developmental assessments, including but not limited to Ages and Stages Questionnaire (ASQ-3) and Modified Checklist for Autism in Toddlers (MCHAT)
  • Maintains a child development support line and responds to and tracks call requests within the designated response time
  • Refer and assist clients with accessing, navigating, and connecting with services within JRCHC and community resources and services.
  • Ensure linkage of children referred to Early Intervention and CPSE
  • Provide anticipatory guidance to parents and other primary caregivers around common concerns such as early learning, fussiness, sleep, feeding, discipline, toilet training, etc.

TEAM-BASED CARE*

  • Conduct team-based well-child visits before, during, or following the primary care provider visits focus on monitoring development, social-emotional functioning, and relational health, and providing anticipatory guidance
  • Conduct consultations on a short-term basis for needs regarding development and/or behavioral concerns such as sleep, positive parenting, picky eating, etc.
  • Work in collaboration with clients’ healthcare providers to evaluate the social determinant of health for the patients
  • Ability to demonstrate acumen in medical terms, and understanding of the social determinants of health
  • Collaborate with HealthySteps implementation team to implement all eight Core Components including universal screenings, making positive parenting and early learning guidance information available, and adjusting workflows for optimal efficiency
  • Maintain open communication with all members of the primary care team

PROFESSIONAL DEVELOPMENT*

  • Participate in HealthySteps training (Virtual HealthySteps Institute)
  • Participate in administrative and clinical/reflective supervision meetings
  • Attend both Behavioral Health and Early Childhood Development team meetings
  • Attend continued education CEU courses

ADMINISTRATIVE*

  • Maintain extensive databases required to meet HealthySteps fidelity metrics including both internal and external referrals.
  • Ability to interpret, adapt, and apply guidelines and procedures to inform and make recommendations to supervisor
  • Assist in creating and maintaining community resource directory/database. Provides Referrals and track follow-up, as appropriate, to help families make successful connections to key resources within the community.
  • Participates (when appropriate) in community-wide early childhood and/or mental health meetings.
