Job Summary: Under the supervision of the CNO, assist in the implementation of policies and procedures. Work in conjunction with physicians to coordinate care throughout hospital stay and also organize, evaluate and plan discharge needs of patients. OSHA Blood borne pathogens Class I.


ESSENTIAL DUTIES AND RESPONSIBILITIES:


General


  • Implementing policies governing asepsis and infection control.
  • Developing a system for identifying, investigating, reporting, and preventing the spread of infections and communicable diseases among patients and Hospital personnel, including contract staff and volunteers, especially those occurring in clusters.
  • Cooperating with Hospital-wide orientation and in-service education programs.
  • Cooperating with other departments and services in the performance of quality assurance activities.
  • Cooperating with disease control activities of the local health authority.
  • Assist in annual employee training of general safety and health standards.
  • Responsible for maintaining compliance with all regulatory bodies, including Federal, State, and applicable accrediting agencies, i.e. CMS, OHCA.


UTILIZATION REVIEW


  • On call to cover after hours admissions as assigned.
  • Knowledgeable of UR guidelines and rules.
  • Knowledgeable of Medicare, Medicaid, and private insurance payors.
  • Keeps charts up to date with current information on number of days in each status.
  • Communicates with Doctors about appropriate status and when/if the patient needs to change statuses


DISCHARGE PLANNER


  • Keep in contact with nursing staff and Doctors of all discharge dates
  • Education of disease process, medications, services provided at home to patient and family
  • Communicating with Doctor of patients needs at home such as; home health or hospice services and home health products.
  • Documentation of all communications involving patient’s discharge
  • Keep medical records accurate and up to date

OTHER SIGNIFICANT REQUIREMENTS:


  • Maintains confidentiality while working with confidential matters on a daily basis.
  • Provides direct service to patients while maintaining a positive patient relations atmosphere.
  • Interacts with patients, family members, and medical staff on continuous basis.
  • Follows aseptic policies and procedures for using precautions when involved in direct patient contact or handling potentially infectious materials.
  • Follows safety guidelines and policies for the hospital.
  • Participates in staff education and attends mandatory meetings.
  • Responsible for own education requirements to comply with Board of Nurse Examiners regulations.
  • Understands and complies with Center for Disease Control and Infections Control guidelines.

ADDITIONAL DUTIES AND RESPONSIBILITIES:

  • Performs other duties that may be necessary or are in the best interest of the company as assigned by the CNO.


WORKING CONDITIONS AND PHYSICAL rEQUIREMENTS:

Work performed in an office and hospital settings.

Subject to long, irregular hours.

Must be able to lift up to 50 lbs.

Regularly required to sit, stand, bend, reach and move about facilities

The noise level in the work environment is usually moderate.

May be exposed to virus, disease, and infection from patients and specimens in working environment.


Knowledge, Skills, and Abilities:

Possesses skills and knowledge in the following areas:

  • Theories of nursing
  • Regulatory requirements
  • Principles of patient satisfaction
  • Strong interpersonal skills with effective oral and written communication skills
  • Knows and practices the requirements of the Board of Nursing Examiners Act


Education and Experience:

Graduate from an accredited school of practical nursing.

Registered nurse currently licensed in the State of Oklahoma

One to Two years of experience in clinical nursing.

Current BLS, ACLS and PALS

Must be qualified through education, training, experience, and certification or licensure to oversee the infection control program.
