This position is a Homecare based role in the Livingston and Oakland County areas with a $2.00 per hour additional incentive. This incentive is subject to change or elimination based on regional homecare needs.

Job Summary
:
Performs patient care activities, including maintaining optimal physical and emotional comforts toward achieving maximum self-reliance and independence within the Day Clinic or participants homes. Activities will include providing basic ADLs, documentation, reporting changes in participants conditions, and other tasks as assigned to assist the primary care, therapy, and healthcare staff. Works under the direct and indirect supervision of the Center Manager but is independent in the application of nursing knowledge and skills.


Skills and Knowledge

  • Must possess the skills necessary to provide care for geriatric patients and must be able to function well in a team environment.
  • Must possess the ability to establish and maintain interpersonal and interdepartmental relationships.
  • Must have the ability to problem solve and interpret instructions.
- Abides by all established Huron Valley PACE policies, rules, and regulations.
  • Only act within the scope of his or her authority to practice.
  • Meet a standardized set of competencies for the specific position description established by Huron Valley PACE and approved by CMS before working independently.

Required Qualifications
1. High School Diploma or GED required.
2. Certified Nurse Aide Certification in the State of Michigan
3. CPR certification (or willing to obtain)
4. Must have reliable transportation, acceptable driving record, and valid drivers license
5. Minimum of one (1) year working with the frail and elderly
6. All employment offers are contingent upon successful completion of a pre-employment criminal background check, verification of professional licensure, if applicable, and TB Screen at time of hire. In addition, proof of COVID-19 vaccination or approved exemption is required.


Desired Qualifications

1. Homecare experience a plus
