Job Summary: In a partnership with Foundation Communities, the Clinical Case Manager will provide psychosocial rehabilitative and skills training services to adults diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective. Case Managers operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams and cultivate a clinical skill set by forging therapeutic relationships with diverse community members. Services will primarily be provided on-site at Foundation Communities single adult studios and will require the use of a personal automobile.

Case Managers work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. Case Managers participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Case Managers work with individuals in developing skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as medical care, nutritional assistance, financial assistance, transportation, legal assistance and resources fulfilling any basic need. Case Managers may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities. Individuals served at this level are relatively stable with limited need for crisis intervention. Although staff members in this position are not typically LPHA's, staff members in this position are working under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment.

Services are provided through a trauma-informed and person-centered lens, striving to promote a more balanced life and are not solely focused on symptom reduction.

Reasonably regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Works under general supervision with moderate latitude for the use of initiative and independent judgment.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.
Preferred:
  • Graduated from an accredited college or university with a Master's degree with a major specialization in Human/Behavioral/Social Services or related field.
License:

Required:

  • Current and valid driver's license and a clean driving record.
  • Center certification (QHMP-CS) to perform case management.
Hours: Full-time: 8:00am - 5:00pm; Workdays: Monday - Friday


Salary
: $22.35 - $23.47 an hour based on education. "Internal applicants are subject to Integral Care's QMHP career ladder".
