  • PROGRAM/DEPARTMENT RESPONSIBILITIES:
  • Confirm with DHCD the family's arrival at program.
  • Explain the program to entering families.
  • Facilitate the intake process and accumulate information to assess the strengths and needs of the family – initial service assessments.
  • Within 10 days of the family's entry into program, draft the family's initial strength-based self-sufficiency plan.
  • Develop an ongoing support system for each family utilizing available community resources and services to assist the family in meeting the requirements of its self-sufficiency plan.
  • Meet weekly with the head of the family to monitor, encourage, and assist in compliance with the self-sufficiency plan.
  • Document and report to DHCD and YMCA the family's progress in implementing its self-sufficiency plan.
  • Provide feedback to DHCD and YMCA concerning issues and barriers faced by the family regarding the self-sufficiency plan by attending regular case conferences scheduled by the DHCD for the family.
  • Monitor, document, and report to DHCD and YMCA on each family's compliance with program rules and regulations.
  • Provide intensive housing search for each family that has entered program.
  • Oversee rapid re-housing of family by assessing the family's progress in meeting its housing search obligations as defined by DHCD and to address any barriers to obtaining and maintaining permanent housing.
  • Act as an advocate for families with government and social services agencies.
  • Accompany family to meetings, appointments, etc., as needed to provide advocacy, explanation and support.
  • Ensure healthy development of children by developing child treatment plans and make referrals to community agencies as necessary (work in partnership with Child Case manager).
  • Periodically review the rules/regulations and violations policy of the program with each family.
  • Work as part of staff team to foster a harmonious atmosphere within the program by helping residents to identify and resolve social and other problems.
  • Attend and actively participate in Family Services activities, groups, special events, and dinners as needed to develop healthy relationships with families.
  • Work as part of a staff team to develop family care planning goals as part of a clinical case review team.
  • Works as part of staff team to provide intensive support services to all families residing at the FLC, the DJL and for all graduated families in the community.
  • Maintain up-to-date client records in locked file cabinet.
  • Facilitate monthly audit discussion group focusing on a topic pertinent to family life while in the program and/or after the family returns to independent living.
  • Participate in staff meetings and trainings as requested by the Director.
  • Other duties as assigned by supervisor.
Requirements:

EDUCATION/EXPERIENCE/PHYSICAL REQUIREMENTS:

  • Degree in Human Services or related degree preferred.
  • Minimum of five years experiencing in human services or residential setting.
  • Valid Driver's License, proof of insurance and driving record.
  • Experience with in crisis youth and families
  • Experience w/Microsoft office suite. Suite includes word, excel, access and PowerPoint.
  • Additional skills should include grammar, punctuation, spelling and business writing.
  • Ability to lift 50 lbs on occasion.
  • Must be able to walk, sit, stand, bend, or stoop, and walk up and down stairs as needed during the work day.
