The Position: We are looking for a Case Manager, who is willing to embrace McKinley’s mission and H.U.M.A.N values. The role of the Case Manager is to work in concert with the treatment team and particularly the STRTP Clinical Social Worker to liaison with the residents' families and county workers with regards to the progress and treatment issues of the individual clients. The Case Manager will assist in coordinating the Needs and Service Plan (NSP) and treatment/case conferences.


Compensation and Benefits:

The pay range we’re offering is $20.00 - $25.00 Hourly depending on experience.

Qualifications:

  • Bachelors degree in behavioral sciences, human services or social services fields.
  • Child welfare and/or case management experience is preferred
  • Bilingual in English/Spanish
  • Proficient in Microsoft Office Suite (MS Word, Excel, PowerPoint, Outlook) and other office-related software
  • Possess superior oral and written communication skills as well as interpersonal skills
  • Detail oriented and organized
  • Able to work independently and organize multiple projects with minimum supervision
  • Able to work as a team member
  • Able to maintain the highest level of confidentiality and professional boundaries with clients and staff
  • Able represent the agency professionally and appropriately with all constituencies
  • Must pass pre-employment physical exam, TB and drug screening
  • Ability to commute to various sites on and off campus
  • Ability to lift 10 pounds for filing
  • Must complete crisis prevention/intervention training (Pro-ACT) within the first 90 days of employment

Key Responsibilities:

  • Maintains a caseload of clients which they follow from admission to discharge
  • Complete appropriate documentation.
  • Provide coordination of sponsorship applications from beginning to completion.
  • Coordinate with the Federal Field Specialist on the process from intake to discharge.
  • Responsible for contributing to the implementation of individual service plans.
  • Ability to implement case plans through good professional judgment while maintaining a team approach is essential
  • Liaison with the residents' families and authorized representatives with regards to the progress of transitioning from shelter care.
  • Attends assigned trainings (20 hours per fiscal year)
  • Other duties as assigned.
