Position Purpose:

The primary purpose of your position is to provide coordination/leadership of direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by charge nurses and the certified nursing assistants (CNA’s). Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing and Health Center Administrator to ensure that the highest degree of quality care is maintained at all times.

Areas of Responsibility:

Supervisory Duties

  • Manage all supervisory duties for the unit, overseeing registered and licensed practical nurses, CNA’s and support staff.
  • Set work schedules, delegate assignments, assign tasks and evaluate employee job performance, as well as disciplining employees who don't fulfill their job requirements or provide inadequate patient care.
  • Mentor, coach and support unit staff including CNAs and nurses, offering clinical and career advice.
  • Set goals and standards for the unit, and hold regular staff meetings for the unit in which the CTM gives directions, offers education or discusses areas for improvement.
  • Assist with infection control documentation and preventative measures.

Clinical Duties

  • Ensure the facility standards of nursing care are met on the unit.
  • Monitor resident care to ensure it meets the facility's standards, and review resident records to evaluate documentation and analyze the effectiveness and efficiency of the care provided by staff on the unit. If a nurse has a question or concern about a patient's care, the CTM may act as a facility resource or may consult the resident’s physician for recommend treatment options.
  • CTMs not only monitor overall care, they may review an individual resident’s case, especially if it's complicated or if the resident is not responding to treatment.
  • Address questions or complaints brought forward by residents or their families.

Administrative Duties

  • Contribute information that helps facility leadership to establish budgets for personnel and supplies.
  • Ensure the unit is appropriately stocked with medical supplies, and oversee management of medications and equipment.
  • Participate in the process of interviewing and hiring employees, using the Applicant Tracking System.
  • Create and deliver training and staff development programs.

Additional Duties

  • Follow established policies and procedures and perform duties according to all applicable regulations.
  • Utilize equipment/supplies in a safe and effective manner.
  • Perform work timely and efficiently.
  • Work cooperatively with co-workers, supervisors, families, and other departments.
  • Create and maintain warm and positive atmosphere, including professional conduct at all times.
  • Follow infection control principles/policies.
  • Direct day-to-day functions of the nurses and CNA’s in accordance with current rules and regulations that govern long term care.
  • Review and resolve staff, resident and family complaints.
  • Cooperate with other resident services/departments when coordinating plan of care.
  • Complete accident/injury reports as necessary. Monitor falls and other resident- related risk factors and develop preventative measures/programs.
  • Oversee the admission & discharge procedures for all residents on your unit.
  • Supervise and assist with training new employees.
  • Make frequent rounds to ensure that nursing personnel are performing their work assignments in accordance with acceptable nursing standards. Monitor daily staff performance and conduct performance evaluations and disciplinary measures as required.
  • Ensure nursing areas are clean and maintained (nurses station, medication cart, treatment cart, medication room, and supply rooms).
  • Ensure that adequate stock levels of medication, medical supplies, equipment, is maintained on your unit, at all times to meet the needs of the residents.
  • Review resident care plans for appropriate resident goals, problems, approaches, and revision based on nursing needs.
  • Maintain confidentiality of all resident information.
  • Ensure that all care is provided in privacy, and that all nursing personnel knock before entering a resident’s room
  • Assess resident’s medical condition, facilitate, and direct treatment.
  • Perform additional tasks as required/requested.

Working Conditions:

  • Works at nurses station as well as throughout the nursing service areas
  • Sits, stands, bends, lifts and moves intermittently during work hours
  • Is subject to frequent interruptions
  • Is involved with residents, personnel, visitors, government agencies, under all conditions and circumstances
  • Is subject to hostile and emotionally upset residents, and family members, personnel, and visitors
  • Communicates with medical staff, nursing staff, and other departments
  • Works beyond normal working hours, and in other positions temporarily, when necessary
  • Attends and participates in continuing educational activities
  • Is subject to exposure to infectious waste, diseases, and hazardous materials

Minimum Qualifications:

  • Must be a licensed RN in the state
  • Long-term care experience
  • Must be able to read, write and speak the English language
  • Must be able to relate to and work with residents who are ill, disabled, elderly, hostile, and emotionally upset at times
  • Must deal tactfully with staff, residents, family members, visitors, and state/federal officials
  • Must have patience, tact, able to deal with stress, and willingness to handle difficult situations and residents
  • Must have working contact phone number

Physical Demands:

  • Must be able to push, pull, move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.

  • Must be able to stoop, bend, and squat occasionally
