Bilingual Family Assessment Worker - Healthy Families (Staten Island) Job at The New York Foundling

The New York Foundling Staten Island, NY 10304

  •   Initiating contact to screen families in the community who are prenatal or have a child under 3 months
  •   Initiating contact with families who have been referred to Healthy Families Staten Island to conduct a formal assessment, preferably in their home, to determine the needs of the family
  •   Advocacy and linkage of families to appropriate services in the community
  •   Establishing a trusting relationship with families
  •   Advocacy and linkage of families to appropriate services.
  •   Keep required tracking forms and maintaining weekly documentation, including screenings, assessments, data forms, and completed weekly outreach
  •   Conducting intensive community outreach to engage families in services including presentations to staff of community agencies and groups
  •   Establish partnerships with community-based organizations and other community groups
  •   Program presentations to staff of community agencies and groups to encourage referrals
  •   Create and maintain an extensive outreach plan to support enrollment
  •   Write up formal Assessments and submit to Supervisor for review within 48 hours of completed assessment visit
  •   Maintaining up to date case records for each family. Maintain clear, concise and accurate documentation of progress that promote peer and supervisory review and feedback, and that demonstrates compliance with model and meeting our agency’s expectation. Assure documentation (data entry) for contacts, incidents, and reports are submitted timely (as indicated by program).
  •   Attending required initial and ongoing training within New York State, agency training, and program level training. Some trainings require outside of Staten Island traveling.
  •   Receiving and attending supervision from NY Foundling leadership. Attend mandatory weekly coaching and mentoring on-site or off-site at identified location by the supervisor.
  •   Attending staff meetings and team-bonding opportunities
  •   Maintaining confidential stakeholder/client records.
  •   Willing to cover for team-member(s). Willing and able to work flexible hours. Some evenings and weekends required.
  •   Related duties as assigned by supervisor or administration to ensure that program outcomes are being reached.

  •   Any other duties and/or tasks that may be assigned.

Education, experience and special needs required: Bachelor’s Degree

AA Degree or equivalent with 3 years of experience in the field – could be accepted

Bilingual Required

Valid NYS Driver’s License

  •   Experience and willingness to work with the culturally diverse populations that are present among the program's target population
  •   Knowledge of infant and child development
  •   Open to reflective practice (i.e. has capacity for introspection, communicates awareness of self in relation to others, recognizes value of supervision)

Scheduled Work Hours Per Week: 40

Schedule: M-F 9am to 5pm; flexibility required

Job Type: Full-time

Pay: $44,000.00 - $51,300.00 per year
