Job purpose

The Assistant Program Coordinator works with the Program Coordinator to provide person-centered and strengths-based support to participants of Ralph Moses House Transitional Housing/Rapid Rehousing Program. The program assists individuals with locating and acquiring appropriate housing and provides ongoing social service supports to promote stable housing. The Assistant Program Coordinator will work with the Program Coordinator in providing the needed social service supports to help program participants achieve their goals and in managing the facility

Duties and responsibilities

Program Management

· Assist Program Coordinator with scheduling & shift coverage

· Oversee the use of the program logbook

· Maintain caseload of up to 10 Transitional Housing program participants, which requires:

o Weekly case management

o Intensive, participant-centered budgeting and financial goal setting

o Workforce and education supports as identified by participant

  • Accessing public benefits; connecting to health, mental health, and substance abuse care, and other support

o Learning landlord and tenant rights and helping participants advocate for themselves.

· Maintain relationships with landlords in the agency’s network and actively recruit new landlords.

· Ensure safety of all participants and staff

Record Keeping

· Document all significant participant contacts in HMIS.

· Ensure participant files are current and in good order.

  • Conduct participant intakes.
  • Document emergencies and incidents using appropriate communication channels, as needed.

Miscellaneous

  • Assume primary responsibility for the medication stored on site
  • Participate in regular supervision meetings with the Program Coordinator
  • Participate in external meetings and trainings as required
  • Other duties as assigned

Qualifications

· Master’s degree in a social service related field with two years of relevant work experience.

OR

· Bachelor’s degree in a social service related field with three years of relevant work experience.

OR

· High School Diploma or GED with at least five years relevant work experience.

· Demonstrated knowledge of homelessness, housing program and procedures, behavioral health, success planning, community rehabilitation and case management. Knowledge of program management and development procedures.

  • Knowledge of community resources in Delaware County.
  • A sensitivity and aptitude for working with individuals experiencing homelessness.
  • Demonstrated effective problem-solving ability.
  • Proficient computer skills with Microsoft Office applications (Excel, Word, Power Point).

· Must understand relationship-based service models and be comfortable working with community partners.

  • Positive attitude, flexibility, and sense of humor!

Clearances required before employment start date:

· Pennsylvania Child Abuse Clearance,

· Pennsylvania Criminal History Report, and

· FBI Fingerprinting.

Required after employment start date:

· Act 31 Training (completed within 30 days of hire)

· Renewal of clearances as required by law

· CPR/1st aid

Job Type: Full-time

Pay: $35,000.00 - $37,500.00 per year
