A valid Registered Nurse license and a Bachelor's degree in Nursing, two (2) years of Nurse manager or ADON experience, long term care MDS experience and certification preferred; or combination of education and experience. Incumbent should be able to apply common sense understanding to carry out instructions furnished in written and oral form, possess high-quality verbal and written communication ability, strong customer service and interpersonal skills. Must be proficient in the use of various computer applications and be knowledgeable on all state and federal health regulations.

This position is primarily responsible for assisting the Director of Nursing in the overall operations of the Nursing Department in accordance with current state and federal regulations. Additional duties may include completion of the MDS assessments and writing individualized care plans for residents. Must be competent in understanding, and utilizing the QAPI process.

  • Delegates responsibilities, duties, or activities to designate nursing staff when appropriate. Follows up on all delegated responsibilities to ensure completion.
  • Ensure staff carries out resident care policies on a daily basis.
  • Creates written or oral recommendations to the DON concerning the operations of the Nursing department for areas of improvement.
  • Conducts daily rounds of all nursing units to ensure the nursing staff is providing the highest level of care to residents.
  • Assists in reviewing and identifying all new resident medical needs. Makes recommendations on resident needs and documents in resident chart or file.
  • Assures that the Nursing department is maintained in a clean and safe manner for the highest level of resident comfort and satisfaction.
  • Participates in quality assurance performance improvement (QAPI) committee meetings to identify issues and to develop and implement appropriate plans of action to correct identified problems.
  • Participates in infection control programs.
  • Assist with interviewing recruitment and on-boarding of new nursing staff.
  • Participate in wound treatment, and rounds
  • Establishes the assessment reference data for all Minimum Data Sets under the Medicare Prospective Payment System to capture the highest resident utilization guideline levels.
  • Coordinates the need for next billable MDS with the third-party billing in the Finance department.
  • Meets with families and the Director of Nursing, resident, social workers, nursing staff, dietary, activities, etc. to collect medical, hospital, social, dietary and family records of each newly admitted resident.
  • Inputs MDS, reentry, and discharge tracking data in to the computer in an accurate and efficient manner.
  • Gathers all initial utilization data and reports results at the Utilization Data review meeting.

Complies with all CMS guidelines for the completion of the Minimum Data Set.

  • Establishes the assessment reference data for all Minimum Data Sets under the Medicare Prospective Payment System to capture the highest resident utilization guideline levels.
  • Coordinates the need for next billable MDS with the third-party billing in the Finance department.
  • Meets with families and the Director of Nursing, resident, social workers, nursing staff, dietary, activities, etc. to collect medical, hospital, social, dietary and family records of each newly admitted resident.
  • Inputs MDS, reentry, and discharge tracking data in to the computer in an accurate and efficient manner.
  • Gathers all initial utilization data and reports results at the Utilization Data review meeting.
  • Completes the required Medicare assessments on a 5, 14, 30, 60, and 90-day schedule and at change of status, annual reviews. Reviews Medicare assessments on a quarterly basis or as required by the Director of Nursing.
  • Monitors changes in the Minimum Data Sets and inputs changes of data in to the computer. Reports changes to Director of Nursing as appropriate.
  • Develops, implements, and maintains an ongoing quality management program for the resident assessment/care plan.
  • Ensures that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties, including civil money penalties, for false certification.
  • Completes all MDS reporting by the set date.

Demonstrates professional development/skills competency in all activities.

  • Participates in at least 12 hours of training per year on the following topics:
    • Following facility policies and procedures
    • Change of Resident Condition
    • Infection Prevention and Control-Wound Care
    • Safety Procedures
    • Activities of Daily Living
    • Resident Rights
    • Resident Behaviors
    • Resident Abuse (required)
    • Dementia Management (required)
    • Care for cognitively impaired (required)
    • Participates in other in-service, projects, or committees as assigned.
  • Participates in in-service, projects, or committees as assigned.
  • Maintains all certifications necessary for employment.
  • Demonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary.
