Wellness Cntr Coordinator LTC Job at Oakwood Common Retirement Comm

GENERAL SUMMARY:
Responsible for the care management of Oakwood Common residents and the administration of the Lifeline System.
ESSENTIAL DUTIES:
1. Conducts routine assessments of residents and/or when requested by others. Maintains a general understanding of the health of the residents. Keeps physician and family members current on problems or changes in a resident's health conditions.
2. Coordinates and evaluates resident incidents, education and wellness programs.
3. Assures that residents receive home care service or courtesy observations when they are experiencing short-term illness.
4. Provides nursing services to residents for their routine health needs. Responsible for assessing ancillary charges for all related services.
5. Assists Clinical Care Coordinator/Nurse Manager in all areas.
6. Manages Lifeline system including response time and results.
7. Maintains effective relationships and open communication with residents, families, and community staff.
8. Tracks, monitors and communicates location and status of residents in the hospital or skilled facility.
9. Serves as a member of the transition team and responsible for conducting assessments of residents for appropriateness of placement in all areas.
10. Creates and maintains medical records for residents and assures availability to health practitioners who visit residents in the Wellness Center.
11. Monitors residents Observation Reports and follows up as required.
12. Participates in the Level of Care Committee.
13. Works to maintain relationships with Oakwood Health Care System employees.
14. Facilitates and follows-up on employee injury incidents.
