GENERAL SUMMARY:

Under the general direction of the Transfer Nurse Manager, receives and facilitates ED to ED transfers into and out of Beaumont Health facilities. Relays pertinent patient information regarding medical condition to hospital and transport personnel. Escalates transport issues with multiple entities. Records all statistical information for monthly management and quality reports for related transfers as well as transport turnaround times.


ESSENTIAL DUTIES:

1. Facilitate ED to ED transfers into, out of and between all BH acute facilities and standalone EDs. This includes recording into the Transfer Center software and Epic all demographic and patient clinical information. ANYTHING THAT IS A CRITICAL CARE TRANSPORT SHOULD BE HANDLED BY A TRANSFER NURSE – HB ECMO IS GENERALLY IP TRANSFERS.

2. Coordinate with all incoming and outgoing patient transfers with regards to patient transportation, when needed, including the escalation protocol for air and ground transportation. Communicates priority status for Beaumont Mobile Medicine and determines and communicates the level of personnel and equipment needed for transport, working with Transfer Nurses to do so when needed.

3. When using ambulances other than Beaumont Mobile Medicine, provide appropriate information for patient, location and destination, routing instructions utilizing knowledge of geographical area or map book resources and the need for special equipment.

4. Provide relay of patient information to receiving facilities and provide the link, as applicable, between critical care transport teams and transferring or receiving facilities.

5. Enter into Transfer Center software ALL statistical information involving transport requests and vehicle status to be utilized for monthly management reports.

6. Work with BH receiving facility ED staff to coordinate incoming patient locations through communication with triage staff, charge nurses or physicians, as appropriate. This includes activating the trauma page or contacting those individuals who will do so.

7. Communicates with Beaumont Mobile Medicine supervisors and clinical nurse manager issues relative to all vehicles and equipment.

STANDARD QUALIFICATIONS

A. Education:

  • Minimum Basic EMT licensure in State of Michigan.

B. Experience:

  • Minimum 1-year road or emergency room experience. Previous EMS dispatch experience preferred.

C. Licensure, Certification, Registration:

  • None required.

D. Other Skills:

  • Excellent verbal communication and customer service. Proficient with basic computer skills.

WORKING CONDITIONS:

A. Physical Effort:

  • While performing the duties of this job, the employee is regularly required to use hands to type, handle, or feel; reach with hands and arms; and talk or hear. This position is a desk based, either sitting and/or standing for an entire shift. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Uses call center and communications equipment.

B. Work Environment:

  • Normal office environment. Generally good working conditions in an office setting. Frequent interruptions, work pressure and irregularities in work schedule are expected. Employee is subject to continually changing tasks with continually changing deadlines. While performing the duties of this job, the employee is occasionally required to attend both regular and unscheduled meetings.
