Service: Intensive In-Home Team Lead

Summary: The Team Leader must be a Licensed Professional or Associate Level Licensed Professional who has the knowledge, skills, and abilities required by the population and age to be served. Individuals shall have a minimum of 1 year of documented experience with the MH/SA population served. Provides individual and family therapy for each beneficiary served by the team. The Intensive In-Home (IIH) service is a team approach designed to address the identified needs of children and adolescents who, due to serious and chronic symptoms of an emotional, behavioral, or substance use disorder, are unable to remain stable in the community without intensive interventions. This service may be provided to beneficiaries through the age of 20. This medically necessary service directly addresses the beneficiary’s mental health or substance use disorder diagnostic and clinical needs. The needs are evidenced by the presence of a diagnosable mental, behavioral, or emotional disturbance (as defined by DSM-5, or any subsequent editions of this reference material), with documentation of symptoms and effects reflected in the Comprehensive Clinical Assessment or Diagnostic Assessment and the Person-Centered Plan (PCP). This team provides a variety of clinical rehabilitative interventions available 24 hours per day, 7 days per week, 365 days per year. The service is intended to accomplish the following: (1) reduce presenting psychiatric or substance use disorder symptoms, (2) provide first responder intervention to diffuse the current crisis, (3) ensure linkage to community services and resources, and (4) prevent out of home placement for the beneficiary. The team approach involves structured, face-to-face, scheduled therapeutic interventions to provide support and guidance across multiple functional domains including emotional, medical, and health.

Program Responsibilities

  • Designates the appropriate team staff such that specialized clinical expertise is applied as clinically indicated for each beneficiary.
  • Provides and coordinates the assessment and reassessment of the beneficiary’s clinical needs.
  • Provides clinical expertise and guidance to the IIH team members in the team’s interventions with the beneficiary.
  • Provides the clinical supervision of all members of the team for the provision of this service. An individual supervision plan is required for all IIH team members.
  • Ensures intensive case management is rendered by the team to include the following: Assessments, Planning, Linkage and referral to paid and natural supports, Monitoring and follow-up.
  • Assists with crisis interventions.
  • Provides psycho-education as indicated in the PCP (psycho-education imparts information about the beneficiary’s diagnosis, condition, and treatment to the beneficiary, family, caregivers, or other individuals involved with the beneficiary’s care).
  • Responsible for monitoring and documenting the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the PCP.
  • Consults with identified medical (such as primary care and psychiatric) and non-medical providers (for example, the county department of social services (DSS), school, the Department of Juvenile Justice and Delinquency Prevention (DJJDP), engages the community and natural supports and includes their input in the person-centered planning process.
  • Provides and coordinates behavioral health services and other interventions for the child/youth or other family members with other licensed professionals and Child and Family Team members.
  • Participates in the person-centered planning process.
  • Assists with implementing a home-based behavioral support plan with the child/youth and the child/youth caregivers as indicated in the PCP.
  • Provides psycho-education as indicated in the PCP.
  • Ensures NC TOPPS is completed during required intervals, as referenced in the NC TOPPS Implementation Guidelines.
  • Submits documentation for weekly Intensive In-Home team meetings, monthly Child & Family Team Meetings, monthly supervision logs, and client-specific clinical outcomes.
  • Knowledge of how to utilize the agency’s web-based Checkpoint Integrity Support for note generation and addendums.
  • Complete company and state-required trainings.

Minimum Position Reuqirements

  • Masters's Degree from an accredited college or university in social work, psychology, or a related discipline.
  • Licensed Clinical Social Worker (LCSW/LCSW-A), Licensed Clinical Mental Health Counselor (LCMHC/LCMHC-A) or Licensed Marriage and Family Therapist (LMFT/LMFT-A).

*A licensed Professional is an individual who holds a license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession. Licensed Professional who has the knowledge, skills, and abilities required by the population and age to be served. Associate Level professionals actively seeking licensure may serve as the team leader conditional upon being fully licensed within 30 months from the effective date of the DMA Clinical Coverage Policy #8A. For Associate Level licensed team leaders hired after the effective date of the DMA Clinical Coverage Policy #8A, the 30-day month timeline begins at the date of hire.

NOTE: This position must be credentialed by Alliance Behavioral Healthcare and Cardinal Innovations Healthcare MCOs.
