Social Worker Case Manager Bilingual Santa Barbara/Goleta Job at The Wisdom Center

We are excited to be partnering with CenCal to provide a new program called "Enhanced Case Management". In this program we will be reaching out to those community who are at highest risk of institutionalization to provide Enhanced Case Management services.
We are looking for a caring and warm individual, who is good with people, and enjoys working with the elderly to join our team.
This is a hybrid position. You will be working primarily remotely in Santa Barbara/Goleta and will come to The Wisdom Center one time per month for team case consultation. Must have reliable transportation, as you will be meeting members in their residence.
Qualifications:
- Bachelor's degree in a related human services field is preferred, but not required. Previous case/care management experience a plus, and may be considered in lieu of a related degree.
- Bilingual English/Spanish required
- Must have reliable transportation, insurance, clean DMV record. Will use own vehicle to provide in-person services. (Mileage is reimbursed at IRS rate, which is 65.5 cents per mile).
- Must be fully vaccinated from COVID-19. We are a health care facility.
Job Duties:
- Provide outreach to members who have been referred to our program and support them to enroll in the ECM program. This is accomplished through in-person, phone, mail, and telehealth.
- Admit and assess new members.
- Develop care plan based on assessment and implement care management services indicated on plan.
- Attend team case consultation in Santa Maria one time per month.
Job Type: Full-time
Pay: $42,000.00 - $55,000.00 per year
