Responsibilities:

-Check-in and handle intakes

-Evaluate patients’ psycho-social needs and the impact of illness on those needs;

-Evaluate patients’ family/social situation to support a treatment plan;

-Utilize community resources to assist patients’ psycho-social and homeless needs;

-Document history, care plans and prepare reports as needed.

Experience Requirements:

-2 year of experience as a Case Manager.

-2 to 3 years of experience as a Social Worker (BSW). Must have previous experience working with the homeless population. New Grads will not be considered.

Education Requirements:

-Graduation from an accredited university or college with a Bachelor's Degree in Social Work or a related field is required.

Qualifications:

-Current AHA-approved CPR card

Skills:

-Skills in critical thinking, problem-solving, logic and reasoning, judgment, time management techniques and data entry;

-Ability to make improvements or take corrective action;

-Observe, receive, and otherwise obtain information from all relevant sources;

-Organize, plan, and prioritize work;

-Communicate with people outside the organization, represent the organization to patients, clients and the public;

-Deal calmly and effectively in high stress situations;

-Develop constructive and cooperative working relationships with others, and maintain them over time;

-Work within a team as well as independently;

-Speak, read, and write effectively as appropriate for the needs of the audience while demonstrating the proper use of grammar.
