The Social Services Resource Coordinator contributes to the administration of care management. Provides non-clinical support to the assessment and evaluation of a patient's social determinants of health needs and requirements to achieve and/or maintain optimal wellness state. Guides patients/families toward, and facilitates interaction with, resources appropriate for the care and well-being of patients.

  • The Social Services Resource Coordinator work assignments are often straightforward and of moderate complexity.
  • The Social Services Resource Coordinator understands own work area professional concepts/standards, regulations, strategies and operating standards.
  • Makes decisions regarding own work approach/priorities, and follows direction.
  • Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

Required Qualifications

  • Bachelors healthcare related degree or 5+ years in a healthcare outpatient setting
  • Experience with Senior population and understanding of needs
  • Bilingual English/Spanish
  • Knowledge and experience in building local and state community resources networks
  • Strong MS office experience
  • EMR experience with strong documentation skills
  • Strong written and verbal communication skills
  • Strong experience in leading and facilitating group classes
  • Ability to work with various levels of interdisciplinary teams to achieve optimal patient care goals and measures
  • Proven ability to create and maintain health and trustworthy relationships
  • Adaptability to a fast paced and ever changing environment
  • Strong organizational skills
  • Flexibility to work from multiple locations based on patient and business needs
  • This role is considered patient facing and is part of Humana Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • Valid state driver's license
  • Reliable and dependable transportation

Preferred Qualifications

  • BSW or MSW (Master's Social Work)
  • Previous Hospice or Home Health experience
