Empath LIFE, a member of Empath Health, is currently seeking a Registered Nurse Case Manager to join our team.
The Registered NurseCase Manager:
- Provide direct and indirect primary nursing care to assigned participants to include center and home care needs.
- Works under the direct and indirect supervision of the Center Manager and/or Senior Staff Nurse and/or PACE Provider, but is independent in the application of advanced nursing knowledge and skills
- Manages complex clinical situations for which he/she is responsible. Supervises both licensed and unlicensed personnel. Directly reports to the Senior Staff Nurse.
Position Requirements:
- Education and/or Experience: Graduate of a school of professional nursing program required. Bachelors of Science Registered Nursing (BSRN) strongly preferred.
- Licensure:
- Current Florida Registered Nurses License
- Valid CPR certification from the American Red Cross or American Heart Association.
- Must have a valid driver’s license, proof of insurance and have means of transportation.
- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
