We're growing, and we're looking for a dedicated, compassionate home health RN Case Manager to grow their career with us!
- home health case management experience
- Initial and ongoing comprehensive assessments of home health patients
- Oversight of OASIS documentation at regular intervals
- Implements/develops/documents the plan of care to ensure quality and continuity of care
- Provides care utilizing infection control measures that protect both the staff and the patient (OSHA)
- Supervises the home health aide and LPN team to ensure quality of service
- Assures continuity of quality patient care delivered with appropriate documentation
- Performs on-call responsibilities and provides on-call service to patients/families, as assigned
- Provides effective communication to patient/family, team members, and other health care professionals
- Adhere to all company policies and procedures.

Minimum Requirements:
- Active Registered Nurse (RN) license
- Complete a criminal background check, DMV check, and reference check
- Valid driver’s license, insurance, and reliable transportation a plus
- Caring, customer service driven nature
Job Type: Full-time
Pay: $70,000.00 - $90,000.00 per year
