Resident Care Coordinator (RCC) Position Summary:

The Resident Care Coordinator (RCC) ensures the quality care of all residents. This position is responsible for promoting and integrating the mission and core values of Charter Senior Living. This position is responsible for daily operations as it relates to quality of care and services. The RCC manages the care staff in AL by scheduling, evaluating, and supporting the frontline care staff while reporting to the HWD.

Resident Care Coordinator (RCC) Essential Duties:

  • Maintains Charter Senior Living Mission and Core Values.
  • Greet, help, and direct visitors as appropriate.
  • Receive fax or phone calls from physicians, pharmacies, families and take appropriate action.
  • Make copies and manage and update resident charts, appointment books, ALIS, and MARS.
  • Coordinate transportation of residents to outside Medical Doctors appointments.
  • Organize and update in-service binder and assists in the management of Relias to assure trainings are being completed timely; if needed will help the Health and Wellness Director prepare for in-person training.
  • Provide support to frontline staff as needed.
  • Active participant in obtaining resident care need updates from care staff to assist the Health and Wellness Director and/or designee in completing assessments/individualized service plans to accurately reflect the needs and preferences of the resident.
  • Assist in updating resident care plans to assure they accurately reflect days/times services are being provided by care staff.
  • Assists with new employee clinical orientation and training.
  • Assist with training and support to new or less experienced assisted living staff.
  • Coordinates and manages staff schedule.
  • Provides backup by performing Caregiver or Medication Technician role as needed.
  • Prioritize tasks to ensure optimum services to residents as requests and needs change.
  • Performs other duties as assigned.

Job Type: Full-time

Pay: From $40,000.00 per year
