Concordia Visiting Nurses is seeking a Registered Nurse Supervisor for a full time position which will be based at our Baden office location.

Since 1998, Concordia Visiting Nurses has provided high-quality skilled nursing and a variety of therapy services to people in the communities we serve.

Our goal is to deliver quality home health services, in good faith, to those who entrust themselves to our care - and to minister to the unique needs of each individual with respect, dignity and compassion.

The Clinical Supervisor – RN is a licensed healthcare professional that is responsible for ensuring that patient care is coordinated and managed appropriately according to state and federal guidelines. Using independent judgement, the Clinical Supervisor – RN actively manages the interdisciplinary team in planning, implementing and evaluating the patient plan of care for appropriateness to each individual patient’s needs to ensure that high quality healthcare is delivered.

QUALIFICATIONS

• Registered Nurse with a current, valid Pennsylvania license.
• OASIS Certified designation preferred
• Five [5] years of progressive experience in a home health care setting preferred.
• Interpersonal, communication and organizational skills.
• Proficient with knowledge of an electronic health record.
• Knowledge of agency policies, both federal and state regulations and third-party payers
• Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state
