Job Summary:

FAR GROUP is seeking for a Qualified Health Care Provider qualified to provide preplacement Physical, TSpot TB Test, Varicella Titer, MMR Titer, Hepatitis B Titer, Hepatitis C Titer with RNA, Urine Drug Screen, Vision Titmus, Occupational Safety and Health Administration (OSHA) Respirator Questionnaire Review, complete blood count (CBC), EKG, Audiogram, General Health Panel (Chem 18 including CBC), Hemoglobin/HgbA1C Panel (Fasting), Pulmonary Function Test, Chest Xray, Chest X-Ray B-Read Interpretation. to 3 medical centers for a year.

· Varicella (Herpes) Titer (Quantity: 250)

· MMR Titer (Quantity: 250)

· Hepatitis B Titer (Quantity: 250)

· Hepatitis C Titer w/ RNA (Quantity: 250)

· Urine Drug Screen Collection (using VA’s ccf) (Quantity: 250)

· Vision Titmus (Quantity: 250)

· OSHA Respirator Questionnaire (Quantity: 250)

· Varicella (Herpes) Titer (Quantity: 250)

· CBC (Quantity: 250)

· EKG (Quantity: 250)

· Audiogram (Quantity: 250)

· General Health Panel (Chem 18 including CBC) 250 $ 0015 Contract Months 1-12 (Quantity: 250)

· Hemoglobin/HgbA1C Panel – Fasting (Quantity: 250)

· Pulmonary Function Test (PFT) (Quantity: 250)

Certification:

· Health Provider License

· Registration at Maryland state.

· RN must be certified in BLS and ACLS
