JOB SUMMARY:

The Program Coordinator of Intermediate Care Facilities (ICFs) ensures that ICFs and/or Waiver operated group homes for individuals, with an intellectual or other developmental disability, are in compliance with all federal, state, local and Agency regulations, policies, and procedures. Supervises all aspects of facility operations; assists with budget development and coordination of the cost settlement process; gathers baseline data and coordinates screening, admission, and discharge of individuals; establishes teamwork with contract professionals including physicians and other medical professionals; coordinates development and implementation of individualized programs; ensures proper administration of medication by staff; participates in staff recruitment, training, and supervision, and monitoring of work schedules to ensure appropriate coverage at all times. Plans own work with supervisory consultation and considerable latitude for decision making. Functions without direct oversight in accordance with standard operating procedures.

ESSENTIAL FUNCTIONS:

  • Coordinate and manage ICF designated residential homes for all federal, state, local and Agency regulations, policies, and procedures assists in all aspects of facility operations
  • Act as a community liaison for stakeholders for gathering baseline data, and coordinate screening, admission, and discharge of individuals
  • Complete meetings with assigned staff to supervise program compliance, delivery of care systems, assist with budget development and coordination of the cost settlement process.
  • Provides oversight and management of assigned staff trainings.
  • Participate in writing and development of regulatory policy and procedures for the program and oversees implementation of individualized plan development
  • Establish teamwork with contract professionals including physicians and other medical professionals
  • Communicate with leadership team program’s needs, strengths, weaknesses, and overall progress

OTHER DUTIES:

  • Participate in the Developmental and Residential Services Leadership team.
  • Attain and maintains trainer certification in Safety Care/First Aid and CPR.

QUALIFICATIONS:

  • Two to five (2-5) years’ knowledge and working with individuals who have co-occurring Developmental Disabilities/Serious Mental Illness and medical complexities.
  • Knowledge of Virginia Waiver and Virginia Department of Medical Assistance Services.
  • Knowledge and experience with quality assurance and regulatory processes.
  • Knowledge and experience with Department of Behavioral Health services.
  • Knowledge in development of processes to ensure compliance and prompt response to regulatory requests
Minimum Requirements

EXPERIENCE/EDUCATION REQUIRED:

Minimum Requirements

  • Bachelor’s Degree in a human service related field with 2-5 years’ experience with individuals who have developmental disabilities (DD), serious mental illness (SMI), and medical complexities

OR

  • Registered Nurse with a valid license to practice in Virginia with 2-5 years’ experience with individuals who have developmental disabilities (DD), serious mental illness (SMI), and medical complexities in a long-term care setting

AND

  • Have a valid Driver’s license without a negative point status
  • Meets the qualifications for a Qualified Intellectual Disabilities Professional QIDP/ Qualified Developmental Disability Professional (QDDP)
  • Supervisory experienced preferred.
