Job Summary

The Program Coordinator supports the health plan’s Clinical Services programs, including Care Management (CM), Behavioral Health (BH), Utilization Management (UM), Community and Social Services, Health and Safety and Long-Term Services and Supports (LTSS). Supports quality improvement and adherence to regulatory and accreditation standards in one primary area but cross covers other areas as appropriate and needed. Interacts with a diverse range of clinical and administrative professionals to coordinate programs; develops and implements tools and workflows; collects data and produces reports to demonstrate program performance. Helps ensure the health plan’s goals of improving member satisfaction, health outcomes and health equity in Cook County.

This position is exempt from Career Service under the CCH Personnel Rules.


Typical Duties

  • Develops and maintains work plans, project targets, timelines, and program activity tracking tools to track key milestones for completion.
  • Develops tools to monitor and track program performance including audit tools, surveys, dashboards, reports, etc. Collects, summarizes, and analyzes program performance data and creates quantitative and qualitative reports.
  • Presents multi-format reports to a variety of audiences, including reports to Health Plan governance committees.
  • Identifies program gaps, problem areas, opportunities and works collaboratively with internal and external stakeholders to generate meaningful, actionable, results-driven program and quality improvement.
  • Documents complex processes; produces and presents workflows; drafts content for policies and procedures.
  • Documents program management activities to demonstrate compliance with organizational, regulatory and accreditation requirements.
  • Contributes to the Health Plan’s annual work plan and evaluation, particularly by prioritizing high impact and achievable goals.
  • Ensures follow-up on assigned tasks, tracks and ensures timely submission of key program documents/reports
  • Engages internal and external partners to effectively coordinate and streamline information flow, service delivery and performance improvement initiatives; maintains key contact lists for programs.
  • Engages members and providers on direct program implementation.
  • Assists in scheduling, developing, implementing, and coordinating logistics for training programs/sessions and meetings including, but not limited to, agendas, handouts, evaluations, on-line meeting resources, recording notes/minutes and tracking attendance.
  • Performs other duties and coordinates special projects as assigned.

Minimum Qualifications

  • Bachelor’s Degree from an accredited college or university is required (Must provide official transcripts at time of interview)
  • Three (3) years of healthcare, social service or professional academic/educational experience is required
  • One (1) year of experience with data collection, analysis or reporting is required
  • One (1) year of experience coordinating programs or projects is required
  • Proficient in Microsoft Office (Outlook, Word, Excel, PowerPoint) is required (A test to validate proficiency may be administered)

Preferred Qualifications

  • One (1) year of full-time experience analyzing data (quantitative and/or qualitative) and use of database software
  • One (1) year of full-time experience in quality assurance or performance improvement in healthcare and/or social service agency
  • Experience working in a Medicaid or Medicare managed care organization
  • Project management certification or training (Lean/Six Sigma, etc.)
  • Previous work experience with a Home and Community Based Service (HCBS) Waiver Program or Long Term Care (LTC)
  • Proficient in Microsoft Visio
  • Bilingual

  • Knowledge of trends in healthcare, managed care, Medicaid, care management, and social determinants of health.
  • Knowledge of medical terminology.
  • Excellent oral, written and interpersonal communication skills and excellent customer service skills to interact with diverse individuals including CountyCare members, staff, vendor partners and leadership.
  • Excellent organizational skills and ability to prioritize, plan, and organize projects and tasks to meet deadlines in a fast-paced and stressful environment.
