The Greenery Center for Rehab and Nursing has served Canonsburg and surrounding communities since its establishment in 1985. We are conveniently located off of interstate 79 next to Canonsburg General Hospital. Our 140 bed center offers short term rehabilitation, long term care, palliative care and respite stays. We are currently seeking a Physical Therapy Assistant -part time 25-30 hours per week to join our team.

Job Responsibilities:

• Provide Physical Therapy services to residents and initiates referrals when appropriate and in accordance with Physical Therapy Department procedures
• Review requests for physical therapy and physicians’ orders
• Evaluate type of therapy most desirable after consulting with physician
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment
• Develops treatment plans by identifying a problem list, including short and long-term goals and methods to achieve identified goals
• Participate in development and implementation of resident assessments (MDS) and care plans, including quarterly reviews
• Coordinates treatment plan with resident, family, and other team members
• Implements and conducts treatment as outlined in treatment plan
• Monitors resident’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician’s orders
• Provides/recommends/fabricate adaptive devices or other equipment; trains resident and caregiver in use or application of orthotics and prosthetics
• Participates in discharge planning
• Develops appropriate home or community programming to maintain and enhance performance of resident in his/her own environment
• Documents results of resident's assessment, treatment, and follow-up. and termination of services
• Ensure that all therapist notes are informative and descriptive of care provided and residents’ response to treatment
• Supervises and directs LPTA’s in implementation of treatment plan
• Supervises students and volunteers in accordance with state regulations
• Supervises Rehabilitation Aides
• Adhere to Physical Therapy Department procedures regarding documentation of Physical Therapy services
• Consistently demonstrates sound judgment in evaluation, planning, implementation, and follow up of resident therapy programs
• Records indirect and billable treatment time according to Physical Therapy Department procedures and submits monthly statistics to billing office
