Job Summary:

Evaluates, plans, directs and administers programs of Occupational Therapy in a home health setting. Plans and conducts OT programs to restore function, prevent disability, and help patients reach their maximum level of independence. Supervises Occupational Therapy Assistants and necessary Essential Functions

  • Provides direct therapy services to patients admitted to home health rehab services (Optional)
  • Evaluates patients, selects and administers assessment techniques, synthesizes information to establish appropriate patient care plan to include necessary modalities, procedures, exercise programs, therapeutic groups and patient education as warranted by a holistic view of the patient's diagnosis, condition, age, and contextual needs of discharge disposition.
  • Gathers and interprets appropriate data from the medical record, physician reports, patient, and/or family as evidenced by documentation.
  • Communicates patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff and coordinates patient's schedule, treatment programs, regarding discharge planning and recommends solutions to patient care problems.
  • Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges.

Physical Requirements:Vision (corrected) adequate for reading. Intelligible speech and adequate language/cognitive skills to perform job functions. Must be able to push patients in a wheelchair or stretcher. Lifting of patients, equipment or supplies will be required up to 20 pounds frequently and 50 pounds occasionally. Able to laterally transfer patients 100 pounds rarely. Sitting, standing, and walking required throughout the day. Job duties sometimes require climbing stairs, kneeling, twisting, bending; on occasion, crouching, crawling and reaching overhead. Must be able to transfer patients. Personal Protective Equipment such as gloves, goggles, gowns, and masks are sometimes required due to possible exposure to hazardous chemicals or blood and body fluids. Must be able to demonstrate any appropriate exercise and activities to patients/caregivers. Work is in a fast-paced clinical environment. The work environment is primarily indoors but occasionally outdoors.Education, Experience and CertificationsBachelors Degree in Occupational Therapy required. NC license for OT required. Basic Life Support for Healthcare Provider (BLS HCP) from AHA required. Position Overview: The Contract Occupational Therapist is responsible for evaluating and treating patients and facilitating rehabilitation and independence. Develops and designs treatment plans for physically, mentally and/or emotionally disabled patients. Occupational Therapists enable, enhance and improve patients' ability to gain independent, productive daily living and working environments.Duties and Responsibilities:

  • Examines and assesses patients' physical, cognitive and learning abilities to determine realistic attainable rehabilitation goals and plan of care.
  • Treatment interventions are based on findings of the evaluation and the plan of care.
  • Selects educational, recreational and social activities that will help individuals learn work and life-management skills within limits of their cognitive and physical capabilities.
  • Reassesses patients' progress and prepares reports that details progress.
  • Documents therapy activities and maintains necessary records in patient files.
  • Provides training and education to caregivers in preparation of discharge.
  • Recommends changes in patients' work or living environments that is consistent with their needs and capabilities.
  • Provide documentation of medical necessity for approval of DME/adaptive equipment and ongoing treatment.
  • 1-2 continuing education courses per year
  • Performs other related duties.

Qualifications:

  • Bachelor's or Master's Degree in Occupational Therapy. National Board for Certification in Occupational Therapy (NBCOT) preferred.
  • Graduate of accredited Occupational Therapy Program.
  • Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and cognitive dysfunctions
  • Knowledge of human behavior and performance, patience and strong interpersonal skills to inspire trust and respect in their clients
