The Nurse Coordinator works under the supervision of a registered nurse and performs skilled nursing services as delegated by the registered nurse.

Responsibilities

  • Verifies the plan of care prior to each visit and provides home visits to the patient as directed by the Registered Nurse in accordance with company policies
  • Updates and maintains plan of care per Washington state regulations for In-Home requirements
  • Reviews all medications and performs a medication reconciliation at each home visit, and properly documents in system
  • Teaches and trains patients and caregivers in learning appropriate self-care techniques with demonstrations as appropriate to ascertain response to training.
  • Collaborates and exchanges patient information with the other care team members, including the PCP or other Physicians offices and the Registered Nurse on Senior Helpers staff to assure patient's needs are being met. Reports to Senior Helpers Registered Nurse as needed for changes in patients’ status.
  • Provides frequent reports to patients PCP or other Physicians and ensures that requested documentation is completed timely
  • Documents home visit within time frame mandated by Senior Helpers policy
  • Stays abreast of nursing trends and knowledge.
  • Participates in Patient Care Conferences as frequently as needed and according to Senior Helpers policy
  • Completion of tasks on a daily basis.
  • Maintain confidentiality in compliance with HIPAA guidelines.
  • Serve as patient advocate.
  • Serve as role model and mentor to other staff and caregivers
  • Performs other duties as assigned.

Qualifications and Education Requirements

  • Graduation from an accredited school with a diploma for an Licensed Practical Nurse or Licensed Vocational Nurse or Registered Nurse.
  • Current, unencumbered license to practice as a Licensed Practical Nurse or Licensed Vocational Nurse or Registered Nurse in State of Washington
  • Current CPR certification.
  • One (1+) year of clinical experience as a Licensed Practical Nurse or Licensed Vocational Nurse or Registered Nurse.
  • Demonstrated knowledge of the appropriate skills for communicating with individuals of all ages. Excellent interpersonal skills including verbal and written communication skills.
  • Demonstrates knowledge of Federal/State rules and regulations, ACHC standards, and other regulatory requirements.
  • Valid Driver's License.
  • Reliable transportation and company required liability insurance.
  • Must be familiar with general use and functions of the computer, such as, usernames, password concepts, internet, email, navigation of computer desktop, including starting programs, using files, and windows, effective use of navigation buttons and tool bars, ability to self-manage online HR services and online training programs.
