Position Summary
Works in a variety of settings coordinating care needs for Disabled Adults across the healthcare continuum. Incumbents ensure patients receive the most efficient and effective care for their particular condition, allowing for improved patient outcomes. Incumbents understand the system of care delivery for patients and communicate with doctors and specialists making sure services and treatments are delivered timely.
All Registered Nurses employed by the Mecklenburg County Health Department are Public Health Nurses. Public Health Nurses, regardless of official title or classification, may provide nursing services during urgent and/or emergent public health events. These services include, but are not limited to, administration of immunizations for various ages and groups, mass medication distribution, and provision of health services in shelter environments.

Essential Functions
  • Utilize professional training and the principles and practices of public health in the provision of health care services to all populations served.
  • Serve as a liaison/point of contact between the clinic and the patient/family for education, counseling, and general communication and coordination of care.
  • Provide ongoing support to clinical staff and external constituencies to ensure operational success.
  • Mecklenburg County Health Department Employees: Respond in cases of urgent and/or emergent events that require public health interventions. Employees may be trained for and assigned duties unrelated to their regular work; work outside normal business hours and workdays and/or in a different work location may be required. Emergency events include, but are not limited to, communicable disease outbreaks, severe weather situations (e.g. snow or ice storms, tornados, hurricanes), natural disasters (e.g. earthquakes), fire, or power outages, and acts of terrorism.

Minimum Qualifications
Experience:
Minimum of two years of nursing case management experience.

Education:
Bachelor’s degree or higher in Nursing from an accredited college. If a degree in Nursing was conferred without a BSN, employment may be possible with the stipulation the incumbent successfully completes course work and exams for Principles and Practices of Public Health Nursing within a year.

Licenses and Certifications
  • Current license to practice as a Registered Nurse (RN) in NC issued by the NC Board of Nursing or another Nurse Licensure Compact State.
  • Certification in Basic Life Support for Healthcare Providers (CPR & AED) by the American Heart Association that is valid for at least 90 days after hire. Maintenance of this certification is required and as funds are available may be supported as part of employment.
  • Valid NC or SC driver's license.
  • May require the ability to obtain and maintain a Mecklenburg County Operator's Permit (COP).
  • May require an Annual Motor Vehicle Record (MVR) check required.
