Seeking Candidates with Social Work Background

*** Offering $25 hourly ***

*** Hybrid work with Field work and at home administration***

** Several Openings throughout New York State**

Position will be hybrid (including conducting home visits) - the home health assessments.

JOB RESPONSIBILITIES

1. Schedule and coordinate the assessment process with individuals, families and providers in the home and/or community.

2. Must be willing to travel throughout the assigned region (when time permits)

3. Contact the individual and/or family/caregiver to schedule the appointment at a time and location most convenient to that person. This will include nights and weekends at the person’s request.

4. Arrange formal interpreter services as needed.

5. Identify if the individual is capable of participating in an interview (full or partial) and if there is a need for an observation. More than one visit may be required.

6. Conduct the interview/observation in person. Assessment cannot be complete without the person being interviewed or observed.

7. Maintain set productivity standards

8. Meet or exceed established monthly productivity standard.

9. All assessments must meet quality standards (e.g. assessment fully completed, individual participated in the assessment, time frames of assessment maintained, etc.) in order to be included in expected productivity count.

**Must be fully vaccinated**

Required Experience:

Minimum of one year post-educational experience working with individuals with intellectual and/or developmental disabilities

Preferred Experience:

Minimum of one year conducting assessments with individuals with intellectual and/or developmental disabilities; Experience in adapting interviewing skills to elicit information from individuals with intellectual and/or developmental disabilities, their families, caregivers and staff, and various clinicians.

Required Skills:

Strong analytical skills; detail and solution oriented; ability to work independently; excellent written and oral communication and presentation skills; and ability to use spreadsheet or database software; Proficient computer skills including, but not limited to, the ability use the following on a daily basis: Web-based applications, Email, Microsoft Word.'
