The Crisis Case Manager provides short term intensive Case Management services to individuals in community placement who are in crisis, or are in need of greater intensity of service than can be provided by ongoing Case Management worker at the present time. Functions as primary Case Management provider at Allwell Behavioral Health Services Crisis Stabilization Unit for those consumers who do not have a Case Manager or, whose worker is in another county, or whose need require a time limited intensive service.

Responsibilities Include:

  • Provides short-term intensive Case Management services to consumers in community placement.
  • Initiates service to clients identified by and at the direction of team leader.
  • Duties include but are not limited to crisis resolution, locating emergency housing, monitoring of consumer status, service brokering, transportation, and advocacy.
  • Terminates intervention as directed by supervisor.
  • Provides Case Management services at Allwell Crisis Stabilization Unit.
  • Provides Case Management services to consumers at Crisis Stabilization Unit who do not have an ongoing Case Management worker, or whose worker is based in another county. Service will terminate upon discharge from Crisis Stabilization Unit or assignment of ongoing worker.
  • Provides short-term crisis-based CPST for clients identified as in need by Crisis Stabilization Unit and Case Management Director. This service may continue after discharge from Crisis Stabilization Unit if clinically indicated.
  • Complies with clinical supervision requirements.
  • Attends 95% of scheduled clinical supervision sessions as verified by supervisor.
  • 95% compliance with QI standards related to supervisory signatures as noted in QI survey results.

Abilities/Skills:

  • Exercise the ability to assess the problems of the consumers, implement and evaluate treatment for client’s solution to their problem.
  • Must have problem-solving skills, knowledge of crisis intervention techniques.
  • Participate in decisions regarding clients’ best interest; utilize various modalities in helping clients resolve crisis by using coordination, linkage, and referral to community resources for successful resolution to crisis.

Education/Experience/Licensure:

  • Bachelor’s Degree in social work, psychology, or related field required, three years of experience preferred.
  • Must obtain CPR, First Aid and Crisis Intervention Training.
  • Must have valid Ohio driver’s license and auto liability insurance
