WHAT YOU'LL WORK ON
- Outreach to patients to complete initial health intake and welcome visits
- Act as an extension of the primary care provider and Care Advisor by partnering and collaborating with interdisciplinary care planning
- Schedule patients for lab results and refill appointments
- Initiate Patient lab letters
- Facilitate access to necessary care by navigating barriers and advocating for the patient; Integrates the primary care team and Specialty Services (i.e. nutrition, behavioral health, medication therapy management), as applicable to support the achievement of health outcome goals with continuity of care
- Supports with Async in Various chat Apps to support clinical questions
- Supports coordination for external referrals
- Medical records requests
- Complete all required documentation in the electronic medical record; making team members aware/collaborating throughout the continuum of care
- Communicate regularly with integrated team members
- Complete forms and prior authorizations for medications.
- Monitor and check assigned Athena boxes and other assigned boxes for coverage.
WHAT WE'RE LOOKING FOR
- Prior Medical Assistant experience in a virtual/telehealth organization focused on preventative care
- Must possess completion of an accredited Medical Assistant program.
- 2 years medical office experience required.
- Nationally recognized/certified as a CMA or RMA Organizations
Finishing Touches (Preferred):
- Call center or triage experience - ideally within a family practice or similar.
- Previous experience in utilization management, discharge planning and/or home health or rehab.
- Previous Medicare/Medicaid experience a plus Knowledge and experience teaching self-management.
- Bilingual (Spanish & English)Fluency in a foreign language.
- Athena Electronic Health Records (EHR) experience
