Position Summary
The Licensed Practical Nurse (LPN)/ Registered Nurse (RN) provides specialized and individual nursing care to assigned people under the in coordination with a Nurse Case Manager and the persons team). The LPN/RN will work cooperatively with the Regional Manager and other direct support professionals to meet the needs of those served. The LPN/RN’s goal is to promote independence and community inclusion for those they support. The LPN/RN must meet the qualifications of a direct support professional and complete all trainings. This is an essential position and the position may not leave their shift until appropriate coverage has relieved them.

Position Responsibilities

Essential Functions

  • Provide quality, person centered nursing level of care to persons assigned, aligned to a person’s Nursing Care Plan and Person-Centered Plan; Ensure all efforts are documented in iCM.
  • Provide support and assistance for all Activities of Daily Living (ADL’s) for assigned people.
  • Provide nursing and other supportive services in a manner appropriate to person served, with a focus on dignity, respect, and independence.
  • Assist in the coordination and work to ensure the persons served receive appropriate medical services (medical, dental, and psychiatric appointments, lab and diagnostic studies, medical and dental procedures, etc.) in a timely manner, including immunizations remain current.
  • Demonstrate positive behavioral management techniques and communicates with persons served and their families in a person-centered, age-appropriate manner
  • Complete nursing logs, progress notes, daily logs, timesheets, individual planning activities, and other documentation thoroughly, accurately and in a timely manner.
  • Facilitate building self-esteem and confidence in those served through positive interactions and building a relationship of trust.
  • Communicate in a timely and professional manner with persons served, their families, coworkers, employers, and other stakeholders.
  • Adhere to DDA and other appropriate policies, procedures, and practices,
  • Attend and actively participate in required training, team meetings and supervision meetings.

Other Functions

  • Keep up to date on current health services for people with intellectual and developmental disabilities through professional development and continuing education.
  • Assist in the ordering of nursing and medical supplies.
  • As needed, provide transportation supports to those served; Operate an agency vehicle or personal vehicle, safely and efficiently.
  • Perform other duties as assigned.

Education & Experience Requirements

  • Must hold a State of Maryland LPN/RN license from the Board of Nursing
  • Minimum of one year plus in providing nursing services for people with intellectual and developmental disabilities or related disabilities
  • Must have valid Maryland Driver’s License with no egregious violations
  • Must be self-motivated, creative, and innovative in approach to providing community integration and community support services
  • Ability to be a team player and responds to changes and/or crisis in a calm and productive manner
  • Knowledge of database and electronic medical records
  • Demonstrated ability in providing support in a respectful manner that meets the various needs of persons served
  • Must be comfortable using technology and have the ability to use email and document outcomes/information in iCM
  • Must complete all required Company and DDA trainings

Physical Demands

While performing the duties of this job, employees are regularly required to stand, lift, bend, twist, and reach in supporting people with activities of daily living. The ability to lift up to 75 pounds is required. Ability to drive a car is a requirement of the position. Occasionally driving a full-size van is required.

While performing the duties of this job, employees are regularly required to communicate through written and verbal methods, read and interpret information; analyze and solve non-routine and complex problems; observe and interpret situations; learn and apply new information or skills; and interact with the community, employees, people served, and others encountered in the course of work.

Work Environment

This position operates in the community at homes of persons served; flexible schedule with on call hours is part of the position. Must be able to work remotely, drive to various locations and travel to an Company location for occasional meetings and trainings.

Work Schedule

This position can be part-time or full-time. Work schedule varies based on the needs of persons served. This position is not eligible for telework.
