LPN (Licensed Practical Nurse) - Assistant House Manager Job at Community Integrated Services, Inc.

Are you a caring, compassionate, and responsible person who enjoys helping others? Community Integrated Services (CIS) is looking for an LPN to join our team as a House Manager!

As an LPN - House Manager you will provide home-based care for three gentlemen in a Nashua, NH residential program. Each of the clients in the home require 1:1 supports, around the clock. Our incoming LPN – House Manager will manage a team of well qualified Direct Support Professionals and they will assume responsibility for house wide operations.

By not being facility-based, the LPN - House Manager must be self-directed and able to practice efficient time management. You will interact with the client's guardian in the course of patient care, so the LPN - House Manager will need to be clinically proficient but will also require strong communication skills and comfort with a breadth of cultures and lifestyles.

Responsibilities:

  • Manage schedules for client support teams
  • Staff training
  • Supervise DSP's while they help the client with their daily living activities, such as building and keeping a day/evening routine, cooking, nutrition education, housekeeping, communication, appointment management, transportation and so many more life enriching activities!
  • Be available, for staff or provider concerns/questions regarding medication, changes in clients’ condition, hospitalization, among other needs
  • Collaborate with CIS Nurse Trainers
  • Direct staff how best to assist in a crisis
  • Collect emergency information as needed
  • Collect information regarding medication errors
  • Quality Assurance of medication administration, per the He-M 1201 rules and regs.
  • Regular contact with program managers regarding any changes or updates for the client
  • Healthcare coordination
  • Coordinating with service coordinators from area agencies
  • Administer nursing care to clients
  • Establish treatment plans
  • Monitor and report changes in patient symptoms or behavior
  • Communicate with collaborating physicians or specialists regarding patient care
  • Educate patients about health maintenance and preventative care
  • Maintain accurate patient medical records
  • Provide advice and emotional support to patients and their family members

Qualifications:

  • Active LPN license in the state of NH in good standing with the BON
  • Experience with highly behavioral clients
  • Leadership experience
  • Familiarity with medical software and equipment
  • Ability to build rapport with patients and their team
  • Strong problem solving and critical thinking skills
  • Ability to thrive in a fast-paced environment
  • Commitment to skill development, independent living, and full community participation for all individuals.
  • Current medication administration and CPR/First Aid Certification required (will train).
  • Completion of Mandt behavior training (will train)
  • A valid NH driver's license.
  • Auto Insurance at or above NH state minimum.
  • Must pass a National and Local criminal background check, BEAS, DMV records check, and required TB testing.

Job Type: Full-time

Pay: $60,000.00 - $75,000.00 per year
